1075657 03/31/2020 Surface Water Per Section V.A.19 of the WDR, there shall be no chronic toxicity in ambient waters as a result of the discharge. Furthermore, Footnote 31 in Section VIII.A.I of the MRP states that "the median monthly summary result is a threshold value for a determination of meeting the narrative receiving water objective..." The narrative receiving water objective was exceeded at receiving water station RSW-002D (R-B) for the month of March 2020. Immediate notification of this exceedance was provided by voice message to Jeong-Hee Lim of the Regional Board on April 3, 2020, at 11:10 a.m. and to Russ Colby and Jeong-Hee Lim via email on April 3. Written notification was provided on April 6, 2020. Compliance was based on two valid Ceriodaphnia dubia chronic toxicity bioassays conducted in March 2020 using grab samples from receiving water station RSW-002D (R-B). The first test was initiated on March 3 using samples collected March 2, 4, and 6. This test failed the TST with 32.5% reproduction effect in the undiluted sample. The second test was initiated March 27 using grab samples collected on March 26, 27, and 30. It also failed the TST for the reproduction endpoint with 28.9% effect observed in the undiluted sample. As a result, the monthly median threshold objective was exceeded. As previously reported, the TRE for RSW-002D (R-B) was already in progress following an accelerated test TST failure in February 2020. As part of the TRE process, Steps 1 and 2 of the TRE were initiated and the testing frequency was increased to monthly beginning in March. In addition, a full phase I Toxicity Identification Evaluation (TIE) was conducted using a composite of the three samples associated with the accelerated test failure. The TIE results did not provide any additional information because the un-diluted composite sample was non-toxic (i.e., passed the TST with -13.7% reproduction effect). 1088876 01/31/2021 Surface Water The narrative receiving water objective was exceeded at receiving water station RSW-002D (R-B) for the month of January 2021. Regional Board staff was immediately notified by telephone at 7:08 a.m. on January 27, 2021, followed by written notification on January 28, 2021. Compliance was based on two valid Ceriodaphnia dubia chronic toxicity bioassays conducted in January using grab samples from receiving water station RSW-002D (R-B). The first toxicity test was initiated on January 5 using samples collected on January 4, 6, and 8. This test failed the Test of Significant Toxicity (TST) with 57.4% reproduction effect in the undiluted sample. The second test was initiated on January 19 using samples collected on January 18, 19, and 21. This test failed the TST for the reproduction endpoint with 72.5% effect observed in the undiluted sample. As a result, the monthly median narrative objective was exceeded. RSW-002D (R-B) is currently the subject of a Toxicity Reduction Evaluation (TRE), which was triggered on February 27, 2020 in response to a single accelerated chronic toxicity test failing the TST. Steps 1 and 2 of the Detailed TRE Workplan were completed in April 2020, and Step 3 (Phase I Toxicity Identification Evaluation, TIE, testing) has continued through December; however, no causative agents have been identified yet. Because a separate TRE was being conducted for the Saugus WRP final effluent and it is the only known point source, the Sanitation District combined the two TRE Workplans to determine the cause of toxicity at both locations. 1074344 02/25/2020 Surface Water Per Section V.A.17 of the WDR, the wastes discharged shall not alter the color of the receiving waters; create a visual contrast with the natural appearance of the water; or cause aesthetically undesirable discoloration of the receiving waters. On February 25, 2020, Los Coyotes WRP Operations staff noticed a green color in the secondary clarifiers and confirmed the presence of color in the effluent discharged. The Sanitation Districts Compliance staff notified Jeong-Hee Lim, Chief of the Municipal Permitting Unit, of the exceedance via a phone call the same day at 4:44 p.m. and submitted written confirmation of this event to the Regional Board on February 27, in accordance with Section VI.A.2.y of the WDR. Staff identified two potential sources: industrial dyeing facilities that discharge dye into the wastewater collection system upstream of the plant. The two industrial dyeing facilities were visited the same day to inspect each site''s operations and autosamplers. However, the aliquots collected by these autosamplers indicated the facilities were not responsible for the color issues at the WRP. At 11:00 a.m. on February 28, staff installed Sanitation Districts'' autosamplers in the sewers downstream of each site. No color was observed in the Sanitation Districts'' autosamplers, which were removed on March 13. The Sanitation Districts will continue to monitor water quality in the effluent. 1063848 06/18/2019 Surface Water Per Section V.A.16 of the Permit, the discharge shall not result in visible floating particulates, foams, or oil and grease in the receiving water. On June 18 at 12:31 p.m., Sanitation District''s staff observed foam coming from the Saugus WRP Discharge Point 001 and traveling to RSW-002D (R-B), and immediately contacted the Saugus WRP Operations staff, who increased the defoamant dose at approximately 3 p.m. At 1:29 p.m. the sampling crew returned to RSW-002D (R-B) and observed that the foam had dissipated. Although the observed foam was episodic in nature, the Sanitation District''s staff will continue to monitor water quality in the effluent and receiving waters, and will continue the normal procedures of conducting visual inspections of the outfall and increasing defoamant dose if foam is observed. This incident was verified at 2:35 p.m. on June 20, 2019, reported to the Regional Board via voicemail at 6:08 a.m. on June 21, 2019, and confirmed in writing on June 25, 2019. 1085761 11/30/2020 Surface Water The narrative receiving water objective was exceeded at receiving water station RSW-002D (R-B) for the month of November 2020. Immediate notification of this exceedance was provided by phone to Jeong-Hee Lim of the Regional Board on November 24, 2020, at 12:12 PM, and written notification was provided on November 25, 2020. Compliance was based on two valid Ceriodaphnia dubia chronic toxicity bioassays conducted in November using grab samples from receiving water station RSW-002D (R-B). The first toxicity test was initiated on November 5 using samples collected on November 4, 6, and 10 This test failed the Test of Significant Toxicity (TST) with 40.8% reproduction effect in the undiluted sample. The second test was initiated on November 17 using samples collected on November 16, 18, and 20. This test also failed the TST for the reproduction endpoint with 46.3% effect observed in the undiluted sample. As a result, the monthly median narrative objective was exceeded. RSW-002D (R-B) is currently the subject of a Toxicity Reduction Evaluation (TRE), which was triggered on February 27, 2020 in response to a single accelerated chronic toxicity test failing the TST. 1080917 09/14/2020 Surface Water On the morning of September 14, 2020, we discovered that during an overnight loading operation a small quantity of calcined petroleum coke powder was released to the Long Beach Inner Harbor. Given the relatively calm nature of the harbor waters between the time of the release and its discovery, the material remained relatively contained in an area approximately 20 yards by 10 yards in size on the water¿s surface. 1076903 04/30/2020 Surface Water Per Section V.A.19 of the WDR, there shall be no chronic toxicity in ambient waters as a result of the discharge. Immediate notification of this exceedance was provided by voice message to Jeong-Hee Lim of the Regional Board on May 7, 2020, at 7:04 a.m. and to Russ Colby and Jeong-Hee Lim via email on May 7. Written notification was provided on May 11, 2020. Compliance was determined using the results of three Ceriodaphnia dubia chronic toxicity tests initiated on April 14, 22, and 28. The first test failed the Test of Significant Toxicity (TST), the second test passed, and the third test failed. The survival and reproduction endpoints in the undiluted receiving water samples for the three tests were 87.5% and 100%, 5.0% and 9.9%, and -11.1% and 24.7%, respectively. As a result, the median monthly threshold objective was exceeded; however, the final effluent passed the TST and was in compliance for April. As previously reported, a Toxicity Reduction Evaluation (TRE) for RSW-002D (R-B) was already in progress following an accelerated test TST failure in February 2020. As part of the TRE process, Steps 1 and 2 and a screening test were completed. These steps did not identify the cause of toxicity; however, the screening test was toxic with 87.5% and 100% survival and reproduction effects, respectively, using an undiluted sample collected on April 13. In response, a Phase I Toxicity Identification Evaluation (TIE) will be conducted using the April 13 sample. 1074394 02/29/2020 Surface Water Per Section V.A.19.a of the WDR, there shall be no chronic toxicity in ambient waters as a result of the discharge. Furthermore, Footnote 31 in Section VIII.A.1 of the MRP states that "the median monthly summary result is a threshold value for a determination of meeting the narrative receiving water objective..." The narrative receiving water objective was exceeded at receiving water station RSW-002D (R-B) for the month of February 2020. Immediate notification of this exceedance was provided by phone to Jeong-Hee Lim of the Regional Board on March 6, 2020, at 12:51 p.m., and written notification was provided on March 10, 2020. Compliance was based on three valid Ceriodaphnia dubia chronic toxicity bioassays initiated on February 11, 18, and 26 using grab samples from receiving water station RSW-002D (R-B). The February 11 toxicity test passed the Test for Significant Toxicity (TST) with a 1.4% effect in undiluted sample, but the February 18 and 26 toxicity tests failed the TST, both with 43.1% reproduction effects in undiluted sample. As a result, the median monthly threshold objective was exceeded. These tests were also part of accelerated testing, following an exceedance of the median monthly threshold objective in January 2020. The February 18 TST failure triggered the Toxicity Reduction Evaluation (TRE) process. The TRE Workplan was initiated on February 27 and results will be provided with future compliance reports as well as the final TRE report. 1096512 08/11/2021 Surface Water The receiving water sample collected at RSW-002 (R4) on August 11 exceeded the calculated 30-day average Basin Plan objective for ammonia. Because the compliance sample for effluent ammonia was collected earlier in the month, a concurrent effluent sample result for ammonia is not available. This incident was verified on Nov 5, and notification was provided to the Regional Board via telephone the same day at 1:58 p.m. Written confirmation was submitted on Nov 10. Although the receiving water sample exceeded the calculated 30-day average Basin Plan objective, the concentration observed was in compliance with the 4-day average Basin Plan objective for ammonia. Ammonia water quality objectives in the receiving water are calculated automatically in the Districts¿ Laboratory Management System so that an alert can be issued if objectives are exceeded. Due to a system error, Districts staff were not notified of this exceedance until the data was reviewed on Nov 5 during routine preparation of the monthly report. Upon confirmation of the exceedance, the Districts reviewed operational records and conditions at the Los Coyotes WRP at the time of the incident and have determined that all treatment processes were normal. On the day of the exceedance, the sampling crew observed that conditions were normal at RSW-002. Ammonia concentrations in the plants secondary treatment effluent on that day were within the typical range for plant operations, which indicates that the receiving water ammonia monitoring results for Aug 11 may have been anomalous and that the receiving water station may have been in compliance with the objective if more samples had been collected and analyzed. Lab staff have updated the computer application that calculates objectives and sends out ammonia exceedance alerts, and they have checked the system to make sure it is working properly. Lab staff also added a spreadsheet check to their data review to confirm and compare results manually. Violation N eSMR Los Coyotes WRP 385612 R4-2015-0124 N
1087636 12/31/2020 Surface Water The narrative receiving water objective was exceeded at receiving water station RSW-002D (R-B) for the month of December 2020. Regional Board staff was immediately notified by telephone at 1:15 PM on January 8, 2021, followed by written notification on January 8, 2021. Compliance was based on three valid Ceriodaphnia dubia chronic toxicity bioassays conducted in December using grab samples from receiving water station RSW-002D (R-B). The first toxicity test was initiated on December 9 using samples collected on December 8, 9, and 11. This test failed the Test of Significant Toxicity (TST) with 23.2% reproduction effect in the undiluted sample. The second test was initiated on December 22 using samples collected on December 21, 22, and 24. This test passed the TST for the reproduction endpoint with 20.2% effect observed in the undiluted sample. A third test was initiated on January 1, 2021 using samples collected on December 31 and January 1 and 5. This test failed the TST for the reproduction endpoint with 44.6% effect in undiluted effluent. As a result, the monthly median narrative objective was exceeded. RSW-002D (R-B) is currently the subject of a Toxicity Reduction Evaluation (TRE), which was triggered on February 27, 2020 in response to a single accelerated chronic toxicity test failing the TST. Steps 1 and 2 of the Detailed TRE Workplan were completed in April 2020, and Step 3 (Phase I Toxicity Identification Evaluation, TIE, testing) has continued through December; however, no causative agents have been identified yet. Compliance was determined using the results of two Ceriodaphnia dubia chronic toxicity tests initiated on May 7 and 19. Grab samples for these tests were collected from receiving water station RSW-002D (R-B) on May 6, 8, and 11 and May 18, 20 and 22, respectively. Both tests failed the Test of Significant Toxicity (TST). The survival and reproduction endpoints in the undiluted receiving water samples were 0% and 22%, 0% and 53.4%, respectively. As a result, the median monthly threshold objective was exceeded; however, the final effluent passed the TST and was in compliance for May. RSW-002D (R-B) is currently the subject of a Toxicity Reduction Evaluation (TRE), which was triggered on February 27, 2020 in response to a single accelerated chronic toxicity test failing the Test of Significant Toxicity (TST) statistical procedure. Beginning in March, as part of the TRE process, the testing frequency at RSW-002D (R-B) was increased from quarterly to monthly. Steps 1 and 2 of the Detailed TRE Workplan were completed in April, and Step 3 (Phase I Toxicity Identification Evaluation, TIE, testing) is underway; however, no causative agent(s) have been identified yet. Grab samples for these tests were collected from receiving water station RSW-002D (R-B) on June 3, 5, and 8 and June 15, 17, and 19, respectively. Both tests failed the Test of Significant Toxicity (TST). The survival and reproduction endpoints in the undiluted receiving water samples were -5.3% and 30.7%, and 0% and 24.5%, respectively. As a result, the median monthly threshold objective was exceeded; however, the final effluent passed the TST and was in compliance for June. RSW-002D (R-B) is currently the subject of a Toxicity Reduction Evaluation (TRE), which was triggered on February 27, 2020 in response to a single accelerated chronic toxicity test failing the Test of Significant Toxicity (TST) statistical procedure. Beginning in March, as part of the TRE process, the testing frequency at RSW-002D (R-B) was increased from quarterly to monthly. 1123165 10/18/2023 Surface Water Per Section 5.1.21 of the WDR, there shall be no chronic toxicity in ambient waters as a result of wastes discharged. The Ceriodaphnia dubia chronic toxicity test initiated for receiving water station RSW-002 on October 11 failed the Test of Significant Toxicity (TST) with a 30.3% reproduction effect and 60.0% survival effect in undiluted receiving water. Dr. Jeong-Hee Lim, Chief of the Watershed Regulatory Section, was immediately notified via voicemail at 4:44 p.m. on October 18, followed by written notification on October 23. High total chlorine residual from Long Beach WRP was initially reported as a potential source of the receiving water toxicity. Upon further review of the Long Beach WRP final effluent chlorine residual monitoring data from October 13, the source of the receiving water toxicity is unknown . Biological observations were conducted immediately, noting the presence of fish and birds, with no negative impacts observed. Two subsequent chronic toxicity compliance samples were taken in October and passed the TST (see details in the Receiving Water section below). This was determined using the results of two Ceriodaphnia dubia chronic toxicity tests initiated on January 9 and 21, 2020; the two tests failed the test of significant toxicity (TST). Immediate notification of the exceedance was provided to the Regional Board via phone to Jeong-Hee Lim of the Municipal Permitting Unit on January 28, 2020, and written notification was provided on January 29, 2020. 1062088 05/23/2019 Surface Water Per Section V.A.16 of the WDR, the discharge shall not result in visible floating particulates, foams, or oil and grease in the receiving water. At approximately 7:15 a.m. on May 23, 2019, Sanitation Districts Operations personnel observed aggregated white foam and a light petroleum-like odor at Pomona WRP receiving water station RSW-001D. Staff increased the defoamant dose by 7:30 a.m., and the foam substantially cleared from the final effluent around 11 a.m. The Sanitation Districts Industrial Waste (IW) staff conducted an investigation and estimated that the foam-causing material entered the Pomona WRP between 12:00 a.m. and 1:00 a.m. on May 23. The Sanitation Districts provided notification of this exceedance as a voice message left for Jeong-Hee Lim, Chief of the Municipal Permitting Unit, at 4:26 p.m. that day, and a written confirmation of this event was submitted to the Regional Board on May 30, 2019. IW staff identified the source of the foam as Master Recycling Center (MRC), which is an industrial waste discharger located upstream of the Pomona WRP. Staff inspected the facility at approximately 10:15 a.m. on May 23, determined that the facility disposed at least one tote (250 gallons) of off-spec car-washing detergents and shampoo products into the sewer system the previous night, issued a Notice of Violation (NOV) of the Sanitation Districts Wastewater Ordinance to the company, and ordered the company to cease discharging these materials. Later that day, the final effluent discharge from the WRP had minimal foam and RSW-001D was largely clear of foam; all indications of foam were gone by approximately 2:30 p.m. the following day. The Sanitation Districts will continue to monitor water quality in the effluent and receiving waters, including routine visual inspections of the outfall, will increase the defoamant dose if foam is observed, and will continue to conduct any necessary inspections of MRC and other nearby industrial waste dischargers. 1069328 10/29/2019 Surface Water On October 29 at 10:39 a.m., Sanitation Districts sampling personnel observed foam coming from the San Jose Creek WRP Discharge Point 001A and traveling to RSW-006 (R-12). This incident was reported via voicemail to Jeong-Hee Lim on October 30, 2019 at 9:47 a.m. and written notification was provided on November 4, 2019. Upon discovering the foam at RSW-006 (R-12), the sampling crew immediately resolved the issue by contacting the San Jose Creek WRP Operations staff, who responded by increasing the defoamant dose at approximately 11:00 a.m. The sampling crew returned to receiving water station RSW- 006 (R-12) at 1:30 p.m. on the same date and observed that the foam was no longer present. The Sanitation Districts will continue to monitor water quality in the effluent and receiving waters and the San Jose Creek WRP Operations staff will continue their normal procedures of conducting visual inspections of the outfalls and increasing the defoamant dose if foam is observed. Upon discovering the foam at RSW-002 (C-2), the sampling crew immediately resolved the issue by contacting the San Jose Creek WRP Operations staff, who responded by increasing the defoamant dose at approximately 11:30 a.m. The sampling crew returned to receiving water station RSW- 002 (C-2) at 1:57 p.m. on the same date and observed that the foam was no longer present. The Sanitation Districts will continue to monitor water quality in the effluent and receiving waters and the San Jose Creek WRP Operations staff will continue their normal procedures of conducting visual inspections of the outfalls and increasing the defoamant dose if foam is observed. On February 28, 2020, Los Coyotes WRP Operations staff noticed a green color in the secondary clarifiers and confirmed the presence of color in the effluent discharged. The Sanitation Districts Compliance staff notified Jeong-Hee Lim, Chief of the Municipal Permitting Unit, of the exceedance via a phone call the same day at 1:42 p.m. and submitted written confirmation of this event to the Regional Board on March 4, in accordance with Section VI.A.2.y of the WDR. Staff identified two potential sources: industrial dyeing facilities that discharge dye into the wastewater collection system upstream of the plant. The two industrial dyeing facilities were visited the same day to inspect each site''s operations and autosamplers. However, the aliquots collected by these autosamplers indicated the facilities were not responsible for the color issues at the WRP. At 11:00 a.m. on February 28, staff installed Sanitation Districts'' autosamplers in the sewers downstream of each site. Unfortunately, based on the timing of the observed color at the Los Coyotes WRP, these samplers were likely installed after the colored waste was discharged to the collection system. No color was observed in the Sanitation Districts'' autosamplers, which were removed on March 13. The Sanitation Districts will continue to monitor water quality in the effluent. 1118443 04/21/2023 Order Conditions Section VII.C.7.c.i requires a statement to be submitted by email to the Regional Board as soon as possible, but not later than twenty-four (24) hours after becoming aware of an unauthorized discharge of sewage or other waste from its wastewater treatment plant to a water of the United States. On April 20, 2023, a spill was confirmed at approximately 10:15 am. Initial notifications were completed as required within 2 hours, including notification to the local health officer and California Office of Emergency Services (Cal OES). In addition, the Regional Board was notified of the spill via voicemail to Jeong-Hee Lim at 6:53 pm on April 20. However, due to an oversight, a written email was sent at 4:25 pm on April 21, after the 24-hour notification deadline. All other spill-related activities, including responding to and stopping the spill, and submitting the required 5-day and 30-day reports, were completed within the required timelines. Chlorinated secondary effluent flowed from the Warren Facility into an LA County flood control storm drain catch basin. The spill then flowed from the storm drain into the Wilmington Drain which discharges to Machado Lake. Sandbag containment was deployed at approximately 4:00 PM to completely stop the spill from entering the storm drain. The total amount of chlorinated secondary effluent that entered the storm drain is approximately 4500 gallons. Per Section 7.1.2.y of the permit, the Discharger shall notify the Manager of the Watershed Regulatory Section at the Los Angeles Water Board by telephone, within 24 hours of having knowledge of noncompliance and shall confirm this notification in writing within 5 days, unless the Los Angeles Water Board waives confirmation. LA Regional Water Quality Control Board staff was notified of the prohibited discharge on November 22, 2023, at 6:00 PM, within 24-hours of receiving the preliminary notification. During this storm, peak flows at JWPCP approached the hydraulic capacity of 675 million gallons per day (MGD), with a maximum flow of 539 MGD. Based on conditions at the time, including the tide height and the level of the water in the surge tower, Operations staff¿s best professional judgment was that temporary discharge of final effluent to Outfall 003 was necessary to avoid a potential plant overflow. The estimated flow rates to Outfall 003 while the gate valve was open on December 30 and 31 were approximately 51.7 MGD and 4.79 MGD, respectively, with a total volume of 35.4 million gallons. All required monitoring was conducted during the discharge event, and all results were within compliance limits. The discharge did not endanger human health or the environment. The opening of Outfall 003 was inadvertently overlooked in the daily summary email from Operations which resulted in the delayed response. On January 3 at approximately 5:47 a.m., the residual chlorine meter in the final effluent exceeded 0.5 mg/L for over five minutes. Operations staff identified the cause as a blockage in the feed line for the sodium bisulfite dechlorinating agent and restored the bisulfite flow at approximately 6:30 a.m. Plant operators collected a grab sample from the final effluent at 7:38 a.m. following the incident and sent it to the laboratory for analysis. Laboratory staff analyzed the chlorine residual concentration in the sample and noted the concentration was non-detect. However, because the laboratory staff erroneously believed the sample was only being used for operational control, they did not record the sample result using their standard protocols. Thus, there is no official documentation of the sample collected January 3 at 7:38 a.m. following the chlorine residual exceedance. To prevent this issue from re-occurring, notification and sample collection procedures have been reviewed with operational and laboratory supervisory staff.
