Both treatment plants were scheduled for inspection on 12/6/2022. However, due the high tides which causes low visibility in the outfall line for east treatment, the inspection of the east treatment outfall had to be postponed. The inspection was rescheduled for the next available time during favorable weather conditions, December 13, 2022. When our contractor attempted to preform that inspection, they determined flow was too strong and they were concerned about the camera becoming detached from the remotely operated vehicle. Based on those conditions a decision was made to reschedule. |

The failure to maintain in good working order a sufficient alternative power source is in violation of section VI.C.4.a of the Order. |

On 9/18/19, a contractor working on a storm water maintenance project at Scripps Institution of Oceanography hit an irrigation line at approximately 1:43pm on the bluff above the beach, west of Hubbs Hall. The water was secured and shut off at 2:02pm. The contractor made every effort to contain the water on the project site, however, from 2:15pm to 2:45pm, some of the water breached the project site through hydrostatic relief holes in a containment structure and discharged to the beach at a rate of 5-10 GPM. It is estimated that a total of 150 to 300 gallons of water discharged onto the beach. Regional Water Quality Control Board staff were notified by email on 9/18/2019 and additional details were provided on 9/20/19. |

On 1/11/22, a non-storm water discharge was observed at Outfall 002 at a flow rate of approximately 0.25 gpm. It was determined to be fresh water using a salinity meter. The source of the water was investigated from 1/11/22 through 1/14/22 and was discovered to be from an underground leak in a main potable water line under the street in front of the Scripps Administration Building. It is estimated that a total of 1,000 gallons of potable water was discharged from Outfall 002 between 1/11/22 and 1/14/22. |

The dioxin subcontractor laboratory (Pace) inadvertently discarded the sample as a result of poor communication and sample management errors between the primary laboratory (Weck) and Pace. Intermittent non-storm water discharges were observed from Outfall 002 on 11/7, 12/13, 12/15, 12/16, 12/17, and 12/18. A source could not be identified through upstream visual evaluations of the storm water conveyance system so the water was sampled and determined to be seawater. UCSD/SIO investigated the seawater distribution system in the area and discovered an underground leak in one of the seawater supply lines. Seawater from this leak got into the storm water conveyance system and resulted in a discharge of approximately 1,000 gallons of seawater to Outfall 002 on five days until the seawater line could be isolated for repair. |

In addition to dilution, a Florisil cleanup was also performed on the Outfall 2 samples for the PCB and Toxaphene analyses. This raised the Method Detection Limits (MDLs) and Reporting Limits (RLs) for these samples. As a result, the MDLs and RLs for select constituents analyzed using EPA Method 625.1 and EPA Method 608.3 for the Outfall 2 storm water samples exceeded Ocean Plan Minimum Levels and/or Performance Goals. The list of MDLs that exceeded the Ocean Plan Minimum Levels and/or the Performance Goals is included in Table 3-3 on page 3-5 of the attached Wet Weather Monitoring report. |
