1143622	05/20/2025	Unauthorized Discharge	Cate School's WWTP had a 400-gallon wastewater spill of raw influent. The majority of the spill went into an open vault for a discharge three-way valve and some spilled onto the surrounding asphalt. Due to the lack of an overflow connection and the absence of remote alarm notifications a spill occurred when the system failed to restart. The incident was not discovered until the onsite personnel arrived the following morning.  Discharger will implement alarm notification system, detailed spill report will be created, evaluations, and investigations will be conducted to determine underlying cause of the phase imbalance that tripped.
