1112350 12/04/2022 Order Conditions This report concerns an unauthorized discharge that occurred at the Cayucos Water Resource Recovery Facility at 800 Toro Creek Road, Morro Bay, California on December 4, 2022. Partially treated wastewater flowed out of our Aeration Basins and Membrane Tanks and ran to a capture basin on-site. All the wastewater was captured and was re-treated. During a period of rainy weather, one of our membrane tanks had a valve that failed to open. That valve failure shut down that Membrane Tank at 00:06 hours. We have determined that the reason for the valve failure was water in the electrical conduit, and have removed the water and re-sealed the conduit. The other two Membrane Tanks onsite were unable to keep up with influent flow, and also shut down, at 00:44 and 00:45 hours. One alarm was sent for each membrane tank failure. These alarms were not acknowledged by the operator on standby. During the time that all three Membrane Tanks were offline the average influent flow was 424 gallons per minute. From 00:45 to 05:28 hours, the flow was 120,541 gallons. At 05:28 hours, influent flow was stopped by turning off influent pumps. These flows were abnormally high for that time of the night due to the ongoing rain event. We pumped the overflow back from the capture basin to our Equalization Basin, where it was then sent back through the treatment system. The water was pumped to the Equalization Basin by end of day, Thursday, December 8th. The retreatment process was completed on December 15, 2022. To prevent a reoccurrence of this unauthorized discharge, we completed the repairs on the Membrane Tank valve on December 8, 2022. We have also changed the number of alarms that the standby operator receives from one for a failure to multiple alarms to ensure proper notification and timely response. A total of 130,785 gallons was recovered and reprocessed through the plant. This total includes washdown water. Violation B eSMR
1106605 06/05/2022 DMON 2nd influent flowmeter failed on June 5, 2022. Total flow data was provided by totalizers, which were still functioning. However, maximum flow rates are unable to be determined for the period of June 5-24, 2022. Repaired flowmeter; installed and sending data on June 25, 2022. Violation B eSMR
