1088113 10/16/2020 DMON he A-8 Colilert sample that was set on Wed, Oct 14th was not read after 24 hours. This was discovered on Fri, Oct 16th. The Colilert tray was clear. This will be documented as being read past required read time but still <1. We are reporting this as "Deficient Monitoring" Retraining for the Operators and Lab staff with specific instruction on paying close attention to dates and set times. Violation U eSMR
1088156 10/15/2020 DMON The A-8 Colilert sample set on Thur, Oct 15th was mistakenly read right after it was set on the 15th. This Colilert tray was never placed back into the incubator because the normal procedure is to toss the tray after reading. Basically the two samples were interchanged. We are reporting this as "Deficient Monitoring" Retraining for the Operators and Lab staff with specific instruction on paying close attention to dates and set times. Violation U eSMR

1073931 08/26/2019 OEV Total Coliform Daily Maximum limit is 23 MPN/100 mL and reported value was 56.5 MPN/100 mL. On 8/21/2019 and continuing through 9/7/2019 a series of coliform tests were positive. The positive hits ranged from 9.7 to 133.3 MPN/100 mL using the Colilert testing method. Once Operations Coordinators were made aware of the ongoing positive tests, a call was made to the RWQCB. In an attempt to diagnose the problem, Operators were directed to begin taking samples in other locations. The samples that were taken at the end of the Chlorine Contact Basin (CCT) starting on 9/3/2019, showed results of <2 MPN/100 mL. The Operators were directed to inspect and clean the effluent wet-well which involved two separate chlorine shocks and draining the wet-well. No evidence of foreign material or fouled sample lines was observed. Arrangements were made to increase the continuous flow from the sample location in hopes of keeping the line clear and providing a representative sample.
