1095503 07/15/2021 OEV Total Coliform 7-Day Median limit is 2.2 MPN/100 mL and reported value was 4.1 MPN/100 mL at EFF-001. Although the exact cause of the exceedance is unclear, the corrective actions were as follows: - The UV disinfection system dose was raised from 120 to 150 mJ/cm². - One UV module was taken out of service for annual maintenance and bulb replacements, and another module was placed in service in its place. - The disk filters upstream of the UV system were inspected. Nine disk panels were identified with tears in them and those 9 panels were replaced. - The UV channel was taken out of service and all modules were cleaned. - The Laboratory Manager has (1) directed all lab staff to notify operations of MPN results greater than 2.2 and (2) set up an automatic email alert in Hach WIMS (the laboratory database) when MPN numbers are greater than 2.2. 1080695 07/30/2020 DMON Total Coliform Other MPN/100 mL. The analyst received a written reprimand that has gone into his employee record, was offered the employee assistance program to deal with any stress that he might be experiencing in his personal life, and an end of the day check log was created for all analysts to ensure they are completing all environmental monitoring requirements and correct bacteria analysis read times. There have been no missed samples since then. Violation B eSMR
1070296 11/09/2019 OEV Total Coliform 7-Day Median limit is 2.2 MPN/100 mL and reported value was 3.1 MPN/100 mL at EFF-001. After the corrective actions taken on November 8th the sample result was <1.0, however, the seven day median remained at 3.1 until 11/10/19. Going forward operations will be notified as soon as possible of any coliform result over 2.2 so that the developed corrective actions can be implemented to prevent any violation in the future. Also Hach WIMS is being reprogrammed with automated trigger points that will notify staff of any results that could adversely affect the 7 day median. Violation B eSMR
1070297 11/08/2019 OEV Total Coliform 7-Day Median limit is 2.2 MPN/100 mL and reported value was 3.1 MPN/100 mL at EFF-001. After notification staff inspected process and discovered damaged filter panels at which point they were replaced. Furthermore, staff disinfected the U. V. channel and increased U.V. dose to 150 mJ/cm2. Laboratory staff has been instructed to notify operations as soon as possible whenever a sample exceeds 2.2 MPN/100ml so that the described corrective actions can be implemented. Hach WIMS is being reprogrammed with automated trigger points that will notify staff of any results that could adversely affect the 7 day median. Violation B eSMR
