Change the planning and management of the monthly samples to subcontract the required testing immediately upon instrument failure to prevent the missed analysis from happening on the originally planned day. 2.Laboratory will plan on performing the monthly samples analysis two times per month to provide redundancy in the laboratory monitoring and reporting program plan. 3. Written reprimand was placed in responsible employee's personnel file. Violation U eSMR
1132985 08/29/2024 CAT1 Carbonaceous Biochemical Oxygen Demand (CBOD) (5-day @ 20 Deg. C) Weekly Average (Mean) limit is 40 mg/L and reported value was 66 mg/L at EFF-001. The resulting increase in inventory in the TFSC system prompted several corrective actions. First, the stalled rotating distributor arm was repaired and placed back in service. Second, the SRT was steadily reduced to increase wasting and remove pounds from the system. Third, the number aeration basins in service was reduced from 4 to 3, to 2 in order to concentrate the inventory into fewer tanks and improve aeration. Fourth, the number of primary sedimentation basins in service was reduced from 6 to 5, to 4, to 3, to bring detention times, surface overflow rates, and weir overflow rates back into standard operating ranges. Last, the entire treatment plant operator workgroup was verbally counseled to follow standard operating procedures, and not make unauthorized changes that could affect process without obtaining guidance from the supervising staff and Contract Chief Plant Operator, and were reminded of the responsibilities and consequences of a failure to meet these duties as licensed treatment plant operators. Violation U eSMR
