Staff determined the cause to be a failed check valve at the utility water pump station. This resulted in recycled water flowing backwards through the system and discharging the facility while the de-chlorination facility remained offline for the diversion. Normally, the TRWQCF supplies the facility with recycled water and is located downstream of the effluent diversion junction box and upstream of the de-chlorination facility. The check valve has since been repaired. |

There was a malfunction with the emergency diversion valves that caused a temporary junction box overflow of tertiary disinfected wastewater. Some of the runoff entered the final effluent flume the rest was captured and sent back to the headworks for treatment. The turbidity spike was due to runoff in the final flume that washed down stream when the plant was brought back online. The cause was determined to be a faulty actuator on the gate. The actuator has since been repaired. |

During the inspection, Board staff observed that the Discharger stores dried biosolids onsite in an unlined area prior to final disposal. This practice may allow contaminated runoff to seep into groundwater during a rainfall event, and negatively impact groundwater. This is a violation of WDRs Provision VI.C.5.b.i, which states in part:
¿the treatment of sludge generated at the Facility shall be confined to the Facility property and conducted in a manner that precludes infiltration of waste constituents into soils in a mass or concentration that will violate Groundwater Limitations V.B. ¿ biosolids on Facility property shall be temporary and controlled, and contained in a manner that minimizes leachate formation and precludes infiltration of waste constituents into soils in a mass or concentration that will violate Groundwater Limitations V.B.¿ |

The wastewater plant was operating normally and the final effluent BOD results prior to the missing day and following the missing day were a 3 mg/L or lower. The highest BOD for the month was a 3 mg/L with the lowest BOD being a 1 mg/L. It is very unlikely that the BOD on 10/15/19 was outside the City¿s effluent limitation range, given the recorded history of compliance. |
The laboratory staff had a corrective action training on 10/18/19 to ensure that all future samples have all of the required analyses completed prior to sample disposal. The training fulfilled the required corrective action for this deficiency. |

We believe that the turbidity spike was due to runoff in the final flume that got pushed down stream when the flow started up. Most likely cause was an equipment malfunction during routine maintenance.
