1121423 09/07/2023 CTOX Chronic Toxicity 1-Hour Average (Mean) limit is 25 % effect and reported value was 27.4 % effect at EFF-001. MRC conducted a chronic toxicity testing on October 4th, which resulted in a "pass." MRC will conduct an additional test on November 6th, which is the earliest availability due to the late notice of the corrected report. Further actions will follow depending on the results. Violation U eSMR
1120795 08/02/2023 CTOX Chronic Toxicity Daily Maximum limit is 50 % effect and reported value was 63.6 % effect at EFF-001. MRC returned to routine monitoring. Violation U eSMR
1119676 07/25/2023 CAT1 Total Suspended Solids (TSS) 24-hour Average limit is 2300 lb/day and reported value was 4069.6 lb/day at EFF-001. QA Lab will come up with a schedule for more routine flushing from the E-001 outflow to the fish lab. Violation U eSMR
1118004 05/14/2023 CAT2 Nickel, Total Daily Maximum limit is 72 ug/L and reported value was 83 ug/L at EFF-001. MRC has increased upstream oily-water separator system cleanings, increased polysulfide addition at the flexicoker unit, and increased nalmet injection at the Flexicoker clarifier. Additionally, MRC is investigating filtration after the selenium precipitation unit, diverting caustic streams to a baker tnak, and filtration upstream of the ETP at the Flexicoker unit. Violation U eSMR
1117240 04/30/2023 CAT2 Nickel, Total Monthly Average limit is 43 ug/L and reported value was 46 ug/L at EFF-001. MRC has increased upstream oily-water separator system cleanings, increased polysulfide addition at the flexicoker unit, and increased nalmet injection at the Flexicoker clarifier. Additionally, MRC is investigating filtration after the selenium precipitation unit, diverting caustic streams to a baker tank, and filtration upstream of the ETP at the Flexicoker unit. Violation U eSMR
1117241 04/14/2023 CAT2 Nickel, Total Daily Maximum limit is 72 ug/L and reported value was 88 ug/L at EFF-001. 1115004 02/27/2023 OEV pH Daily Minimum limit is 6.5 SU and reported value was 6.4 SU at EFF-008. Based on the background rain sample pH readings of 5.9 and 6.0, MRC believes that the storm water pH reading of 6.4 is due to acidity of the rainfall, not contamination. All other readings from this discharge point were within permitted limits. Violation U eSMR

1107617 07/31/2022 DMON On July 29, 2022, MRC sampled for coliform at EFF-001D as dictated by its random sampling schedule. The sample was sent to a contract laboratory, but the driver did not deliver the sample within the 8-hour hold time. MRC was notified of the hold time exceedance on Monday, August 1, 2022. The compliance week as defined in Order R2-2017-0039 is Sunday to Sunday meaning that MRC was not able to take an additional sample to meet the weekly monitoring requirements for coliform for the week of July 24, 2022. The coliform analysis for the July 29 sample was completed but it was not reported in CIWQS due to the qualifier of not meeting regulatory hold time requirements; the result was 2 MPN/100 mL. MRC verbally notified the Regional Water Quality Control Board of the monitoring deficiency on Monday, August 1, 2022. MRC completed an informal investigation to address the deficiency. MRC¿s internal analytical laboratory, which is responsible for managing compliance sampling, has updated their random sampling calendar to ensure that samples are sent earlier in the week to allow adequate time for resampling as necessary. MRC has also requested additional communication from the contract laboratory for sample receipt and preparation as an interim measure until the contract lab completes their investigation and develops and implements corrective actions.
