1121263 08/15/2023 ATOX Acute Toxicity Daily Maximum limit is 2.7 TUa and reported value was 3.2 TUa at EFF-001. All four acute toxicity accelerated samples were taken as required by the Refinery¿s NPDES permit and all four samples were below the permit limit of 2.7 TUa. Violation B eSMR
1118655 05/30/2023 OEV Enterococci Instantaneous Maximum limit is 104 MPN/100 mL and reported value was 2400 MPN/100 mL at EFF-T-710. We are currently investigating the possible source. Additional information and appropriate actions, if any, will be provided once that investigation is complete. Violation B eSMR
1116102 02/27/2023 OEV Enterococci Instantaneous Maximum limit is 104 MPN/100 mL and reported value was 2400 MPN/100 mL at EFF-T-710. We are currently investigating the possible source. Additional information and appropriate actions, if any, will be provided once that investigation is complete. Violation B eSMR
1106607 05/03/2022 ATOX Acute Toxicity Daily Maximum limit is 2.7 TUa and reported value was 3.1 TUa at EFF-001. The refinery has initiated accelerated monitoring as required per NPDES Permit Attachment E V.A.7. The first of four accelerated monitoring tests is being taken within 7 days of receipt of noncompliance results. Thereafter the remaining three accelerated monitoring samples will be taken in 2 week intervals. Violation B eSMR
1103494 02/02/2022 ATOX Acute Toxicity Daily Maximum limit is 2.7 TUa and reported value was 2.9 TUa at EFF-001. The refinery is completing the TRE Workplan as required per NPDES Permit Attachment E V.D. The remaining 3 accelerated monitoring samples have been back in compliance. Additional information as to the possible cause and appropriate actions will be provided as part of the TRE Workplan results. Violation B eSMR
1103495 01/05/2022 ATOX Acute Toxicity Daily Maximum limit is 2.7 TUa and reported value was 3.2 TUa at EFF-001. The refinery is completing the TRE Workplan as required per NPDES Permit Attachment E V.D due to the first of four accelerated monitoring also resulting in acute toxicity above 2.7 TUa limit. The remaining 3 accelerated monitoring samples have been back in compliance. Additional information as to the possible cause and appropriate actions will be provided as part of the TRE Workplan Results. Violation B eSMR
1095649 08/03/2021 OEV Fecal Coliform Instantaneous Maximum limit is 400 MPN/100 mL and reported value was 930 MPN/100 mL at EFF-T-710. We are currently investigating the possible source. Additional information and appropriate actions, if any, will be provided once that investigation is complete. Violation B eSMR
1089220 02/24/2021 OEV Enterococci Instantaneous Maximum limit is 104 MPN/100 mL and reported value was 170 MPN/100 mL at EFF-T-710. We are currently investigating the possible source. Additional information and appropriate actions, if any, will be provided once that investigation is complete. The fifth monthly sample for enterococcus was taken on March 1, 2021 at 3:42pm and results were 3 MPN/100mL. This is below the instantaneous maximum and monthly average suggesting that the source was transitory in nature. Violation B eSMR
1080521 08/03/2020 OEV Enterococci Instantaneous Maximum limit is 104 MPN/100 mL and reported value was 2400 MPN/100 mL at EFF-T-710. We are currently investigating the incident. Additional information as to the possible cause and appropriate actions, if any, will be provided once that investigation is complete. Violation B eSMR
1069646 10/30/2019 OEV Enterococci Monthly Average limit is 35 MPN/100 mL and reported value was 228 MPN/100 mL at EFF-T-710. The refinery is currently investigating the incident. All the enterococcus samples from the month of October have been sent for DNA analysis. The first four results have all come back with detection of bird DNA. There is no human DNA detected in any of the samples to date. The ETP (Effluent Treatment Plant) had low flows during the month of October and we suspect this could have exacerbated the results. Additional information as to the possible cause and appropriate actions, if any, will be provided once that investigation is complete. Violation B eSMR
