1105894 03/08/2022 Order Conditions 500-800 gallons of sewer waste spilled over at the package treatment plant. Spill was caused by faulty pit valves. This is a violation of Board Order No. 6-84-58 Section I.C.1: "There shall be no discharge, bypass, or diversion of raw or partially treated sewage, sewage sludge, grease or oils from the collection, transport, treatment or disposal facilities to adjacent land areas or surface waters." 1098591 11/30/2021 Failure to Notify Bear Valley MHP did not notify Lahontan Water Board staff of a discharge that was discovered on December 1, 2021. This violates Board Order No. 6-84-58 Section II.3. Alpha Omega was contracted by the Bear Valley MHP to conduct an investigation for the cause of the spill and to clean-up the impacted area. During their initial investigation, they took photos of the impacted area and determined the cause of the spill was due to faulty pit valves and the rate and volume of the spill was small. The pit valves were replaced and the spill was contained. Sofia Cambell scheduled Alpha Omega to complete the clean-up of the spill and also remove overgrown weeds in the area on Saturday, December 4, 2021. I (John Yu) provided instructions in a December 7, 2021 email on how to report future spills to Sofia Cambell who is the on-site manager. Violation B Complaint
1094365 09/16/2021 Order Conditions Septic tank from the Bear Valley Ranchos Mobile Home Park treatment system spilled over causing 350-400 gallons of sewer waste to overflow to Bonato Lane and Sioux Road. Violates Board Order No. 6-84-58 C.2. The septic tank was repaired to stop overflow. The sewer waste dried out and Alpha Omega Plumbing & Septic was assigned as a contractor to scrape off the sewer waste for disposal. The clean-up was performed on 09/18/2021 and disposed of by the contractor. Violation B Report
