1127122 04/01/2024 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 49 MPN/100 mL at EFF-001. The contact chamber was drained and cleaned. Adding a new sample port with reduced flow and turbulence should significantly reduce the chance of surface contaminants or turbulence disturbing the sample when grabbed. A small top wall section was removed to facilitate even surface flow and eliminate stagnant water. Violation U eSMR
1125520 03/14/2024 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 33 MPN/100 mL at EFF-001. Monitor facultative pond side stream to headworks. Take additional samples to ensure compliance. Violation U eSMR
1125519 03/11/2024 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 33 MPN/100 mL at EFF-001. We have made air adjustments to the pond and will monitor the conditions. Violation U eSMR
1121637 10/23/2023 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 25.5 MPN/100 mL at EFF-001. This oversite was discussed internally and has been reiterated to be diligent when noting run times and backwash frequency. Violation U eSMR
1116450 04/20/2023 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 36 MPN/100 mL at EFF-001. Increased the chlorine feed rate from 4.00 mg/L to 4.50 mg/L. All other parameters and constituents look good, turbidity has been less than one, free chlorine has been consistently between 1.00 and 2.0 mg/L. We are tracking all parameters to resolve issue. Violation U eSMR
1116451 04/17/2023 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 27 MPN/100 mL at EFF-001.
1114672 02/06/2023 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 25 MPN/100 mL at EFF-001. Working through potential sources that could cause elevated total coliform samples in treatment processes. The ballast pond and chlorine contact chambers were drained and cleaned completely. Total and free chlorine is being tracked during coliform sampling. Also reached out to Pace Analytical lab to confirm all QA/AC is being followed. Violation U eSMR
1113320 01/05/2023 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 130 MPN/100 mL at EFF-001. Increased flows from heavy rain has increased the hydraulic loading on the plant. The sudden increase may have caused a negative effect on the treatment process. Corrective action was to increase the dosage to the contact chamber from 4.0 mg/L to 4.5 mg/L and put an additional pressure filter online to increase filtration efficiency. Violation U eSMR
1113319 01/03/2023 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 130 MPN/100 mL at EFF-001. Increased flows from heavy rain has increased the hydraulic loading on the plant. The sudden increase may have caused a negative effect on the treatment process. Corrective action was to increase the chlorine dosage to the contact chamber from 4.0 mg/L to 4.5 mg/L and put an additional pressure filter online to increase filtration efficiency. Violation U eSMR
1112339 12/22/2022 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 25.5 MPN/100 mL at EFF-001. The soft start to the aerator was replaced and put back in service the following day. Additional samples were taken 12/27/22. Violation U eSMR
1100578 02/03/2022 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 50.5 MPN/100 mL at EFF-001. Spoke with Basic lab about the importance of immediate notification of elevated total coliform samples to allow us time to collect additional samples. The following week we took additional total coliform samples as precaution. All total coliform samples results were 2 or less. Violation U eSMR
1096368 10/14/2021 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 42 MPN/100 mL at EFF-001. Air adjustments were made to our aeration basins and an increase in our chlorine feed was the actions taken to address this increased coliform result. Violation U eSMR
1089855 04/09/2021 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 71.5 MPN/100 mL at EFF-001. We collected additional coliform samples as soon as we was notified of the positive sample. We wanted to demonstrate / confirm we was in compliance with our discharge permit. There was no further violations beyond this specific sample. In addition plant personnel reviewed sample collection protocols and awareness of ambient conditions when collecting samples. Violation U eSMR
1082589 11/02/2020 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 25 MPN/100 mL at EFF-001. Process adjustments made on 11/2/2020 to MLSS,dissolved air concentrations and feed a small dosage of polymer pre-filters. Process came around and effluent has improved. Violation U eSMR
1070667 01/13/2020 OEV Total Coliform 7-Day Median limit is 23 MPN/100 mL and reported value was 23.5 MPN/100 mL at EFF-001. Possible looking at increasing NH3 levels to add chloramines to see if effective. Plant normally runs at 99% complete nitrification, the adjustment is very minor to 0.5 t0 0.9 mg/l NH3 prior to chlorine dosage. Violation B eSMR
