• The discharger failed to submit the 2019-2020 and 2020-2021 Annual Reports
• A Change of Information (COI) needs to be submitted in SMARTS to extend the project completion/end date and recalculate the Risk Level.
• Post-Construction BMP Plan needs to be uploaded to SMARTS.
• Contractor’s list and Training Log need to be updated and included in the SWPPP.
• The construction schedule needs to be updated/included in the SWPPP.
• The discharger failed to perform weekly inspections and observations, and at least once each 24-hour period during extended storm events, to identify and record BMPs that need maintenance to operate effectively, that have failed, or that could fail to operate as intended.

• The discharger failed to establish and maintain effective perimeter controls and stabilize all construction entrances and exits to sufficiently control erosion and sediment discharges from the site.

• The discharger failed to ensure that all storm drain inlets and perimeter controls, runoff control BMPs, and pollutant controls at entrances and exits are maintained and protected from activities that impact their effectiveness.
