SWPPP Violations:
The discharger failed to ensure that all inspections, maintenance, repairs, and sampling activities at the project location were performed or supervised by a Qualified SWPPP Practitioner (QSP) representing the discharger.
The discharger failed to ensure that all persons responsible for implementing the SWPPP requirements were appropriately trained in accordance with the Construction General Permit (CGP).
The discharger failed to use inspection checklist, using a form provided by the State Water Board or Regional Water Board or in an alternative format. The provided inspections lacked key CGP components on their alternative format used.
The discharger failed to update the Risk Assessment for the project when the date for construction completion changed. A new Risk Assessment is required for the revised construction end date. The construction end date identified in SMARTS is November 30, 2019. The permittee needs to complete a revised Risk Assessment.
An updated contractor and subcontractors list was not included in the SWPPP.

Deficient BMP Implementation |
BMP Deficiencies:
Staff observed inadequate perimeter controls throughout the project site. Sediment was observed leaving the site from areas with no perimeter controls or inadequate controls
Staff observed wet and dry concrete wastes and residuals directly on the ground without proper BMPs. No concrete wash-out bins were observed onsite at the time of the inspection
Staff observed sediment leaving the perimeter fence and construction entrance at the north portion of the project site onto the public alleyway
Staff observed inadequate stabilized construction entrance at the north portion of the project site. Mr. Root stated that the entrance was only used a few times and wont be used at all going forward
Staff observed poor Housekeeping Site Management throughout the project site.
