1139645 11/25/2024 DMON On November 25, SCQ collected an acute toxicity sample at EFF-004, which followed the proper sampling technique and occurred under normal ATS operating conditions. On November 26, 2024, discharge ceased from EFF-004. On November 27, SCQ was notified by the laboratory via email, that it did not conduct the acute toxicity test due to using the wrong sample for testing. Therefore, the original sample extended beyond the 36-hour hold time and the acute toxicity test could not be completed. SCQ will retrain sampling staff on proper sampling techniques and will continue to work with the laboratory to ensure samples are processed within hold times. Violation B eSMR

Samples were not collected at the time of the inspection. Stevens Creek Quarry (SCQ) sampled the unauthorized discharge on October 15, 2024. On October 15, 2024, approximately 60,000 gallons total were transferred from Sediment Trap 3 to the ATS, enough to prevent further discharges through outfall EFF-004. No water that was transferred to the ATS was processed or discharged through EFF-004. SCQ plans to review drainage area calculations to evaluate ATS process flow rates and volumes. The discharge observed on October 11, 2024, continued through October 15, 2024. On October 15, 2024, samples were collected, excluding Turbidity and Acute Toxicity from EFF-004. Violation U eSMR
1139640 10/11/2024 Order Conditions An unauthorized discharge of untreated process water from Sediment Trap 3 at Discharge Point 004 during RWQCB inspection was observed on October 11, 2024. The untreated process water flowed to a receiving water (Swiss Creek) through a space between a black, plastic pipe (discharge pipe from a treatment system) and a concrete culvert that the discharge pipe was installed into. The discharge, which was ongoing during the inspection, violated Permit discharge prohibitions III.A and III.B and monitoring requirements in Permit Attachment E, section III.A. The RWQCB sent SCQ an Inspection Report on December 27, 2024, notifying them of the observed unauthorized discharge and a request for corrective actions. Stevens Creek Quarry (SCQ) sampled the unauthorized discharge on October 15, 2024. On October 15, 2024, approximately 60,000 gallons were transferred from Sediment Trap 3 to the ATS, enough to prevent further discharges through outfall EFF-004. No water that was transferred to the ATS was processed or discharged through EFF-004. SCQ plans to review drainage area calculations to evaluate ATS process flow rates and volumes. To prevent reoccurrence, SCQ installed a water pump activation float in Sediment Trap 3. This system automatically activates the pump to ensure untreated water does not discharge into Swiss Creek. Additionally, the surface of Sediment Trap 3 was thoroughly skimmed and cleaned of vegetation, leaves, and floating debris to maintain optimal functionality and water quality. Violation U eSMR
1139648 10/11/2024 Order Conditions The discharge of untreated process water from Sediment Trap 3 affected water quality in Swiss Creek during RWQCB inspection was observed on October 11, 2024. The RWQCB observed sediment in the tributary from Discharge Point 004 and foam and aquatic growth downstream of the tributary that was not visible upstream. Permit receiving water limitations V.A.1 and V.A.4 prohibit discharges from causing floating materials (foams, scum), bottom deposits, and aquatic growths that affect the beneficial uses of receiving waters. The discharge was observed on October 11, 2024, and continued through October 15, 2024. SCQ sampled the unauthorized discharge on October 15, 2024. Additionally, on October 15, 2024, approximately 60,000 gallons were transferred from Sediment Trap 3 to the ATS, enough to prevent further discharges through outfall EFF-004. No water that was transferred to the ATS was processed or discharged through EFF-004. SCQ plans to review drainage area calculations to evaluate ATS process flow rates and volumes. Violation U eSMR
