Deficient Reporting |
Visual observations of the surface water conditions are required whenever a surf zone sample is collected. Surf zone sampling occurred on September 9, 2020 but one of the staff lost/misplaced the Visual Observation Form for that day. |
In response to the Visual Observations Form that was misplaced after the September 9, 2020 shoreline sampling event, the San Elijo Joint Powers Authority (SEJPA) has investigated the incident and instituted corrective actions to make sure this does not happen again. Chris Trees states that in the 11 years that he has been Director of Operations, and the 20 years that Mike Thornton has been the General Manager, this is the first time that one of these forms has been misplaced; thus, SEJPA has properly collected and filed this information over 1,000 times without incident.
A Corrective Action Report was initiated and the investigation recommends that an electronic copy of the form (photo) is created in the field as soon as the last sample is logged and this electronic copy will be texted or emailed to the Laboratory Manager prior to returning to the laboratory. When the staff responsible for sampling has returned to the office, the original hard copy will be brought in with the samples and submitted to the Laboratory Manager for review. Once the hard copy is reviewed and signed in the office, the hard copy will be scanned and saved electronically on the SEJPA computer network. The original copy with signatures will be filed in the laboratory binder and retained in accordance with the document retention policy. |
