Both Phase I and II discharge were in bypass mode for the day at the direction of operations management. Operations staff mistakenly discharged from Phase II for a total of ten minutes in the afternoon; equating to 0.24 MGD discharge, as indicated in attachment B of the eSMR, without collecting a sample for Total Coliform as required by the permit. 1118268 05/28/2023 Deficient Reporting The eSMR is missing a required result at location INF-001 for Conductivity for the week of May 28, 2023. A mix up within the City laboratory led to the sample being discarded before being analyzed. Laboratory staff have been reminded of internal process and policies to track samples as they move through the laboratory. Violation B eSMR
1118269 05/09/2023 DMON On 9 May 2023, TRWQCF staff did not conduct field analysis for the plant influent (INF-001) pH grab resulting in deficient monitoring. Staff was retrained on the monitoring requirements and additional alarms were established. Violation U eSMR
1117512 04/17/2023 DMON On 17 April 2023, the continuous monitoring equipment located at the TRWQCF effluent monitoring station (EFF-001A) was left in hold after routine analyzer maintenance. Unfortunately, both the monitoring analyzers for total chlorine residual and the dechlorination agent was left in hold for 2.5 hours resulting in deficient monitoring with the total residual effluent limitation. In an effort to ensure chlorine was absent, staff coordinated with City of Modesto and intercepted the effluent to an emergency holding basin located at the Modesto¿s Facility. Staff confirmed the absence of total chlorine and then continued pumping to the Delta Mendota Canal (EFF-002). To ensure compliance with this order, the monitoring analyzer standard operating procedure was updated and staff was retrained. Violation U eSMR
1116029 03/28/2023 Order Conditions The eSMR contains a maximum turbidity result over the 0.5 NTU permitted limit. On March 28, 2023 at 04:57:48, the in-line turbidity meter at location UVS-002 reached a result of 0.18 NTU. This result triggered an automatic bypass of the phase II system. In the time it took for the bypass gate to fully close (ten seconds) the in-line meter had spiked at the reported maximum of 2.82 NTU. This is an incontrollable anomaly and the City does not anticipate another occurrence. Violation U eSMR

Operations staff in charge of samples on February 10th believed the plant to be in full bypass and not discharging to receiving water. This lead the operator collecting samples to not take the grab pH sample at location EFF-001B required by the permit. The operator in question has received coaching in this matter and the City does not anticipate a future occurrence. While the city does not have the required grab result, we do have a pH result of 7.23 SU from the in-line meter at location EFF-001B at the same time of sample collection on February 10th. The City has included in this report Attachment G which is a comparison of pH results from grab samples and the in-line meter results taken at the same time as sample collection during the month of February in the hopes that the Board will consider this an acceptable substitution in this case. 1103439 02/01/2022 Order Conditions In February 2022 it was discovered that the UV lamps in bank 1B of phase II were 471 hours over the manufacturer¿s specifications of 12,000 run hours. Since this discovery the City has added a signed daily check of the UV HMI hours of an update by operators to reflect the highest lamp hours. Programing in the HMI was also updated to auto alert SCADA when lamp hours approach 11,800 hours to ensure that the UV lamps are always replaced before exceeding specification. Violation U eSMR

1092661 06/01/2021 Order Conditions On June 1, 2021 at 17:43 a lap in one of the UV vessels hit 12,000 run hours. This went un-noticed until June 2, 2021 at 09:43. During that 16-hour window the flow through the vessel was 635,889 gallons. The lamp was immediately replaced. To avoid these issues in the future, the City of Modesto has implemented a daily documentation of lamp run hours and installed a SCADA system alarm to alert at 11,800 run hours. When the alarm alerts, operations staff will create a work order and immediately notify operations and maintenance supervisors. Violation U eSMR

1082168 10/06/2020 DMON The eSMR for October 2020 does not include Flow results at location INF-001 from October 6-7, 2020. The City of Modesto¿s Ignition SCADA system had an issue with the vendor database server on October 6th, 2020, which interrupted data collection. City IT staff and the City¿s contracted system integrator (Telstar) identified the root cause of the issue to be a missing server license. Telstar purchased the server license initially and after investigating their order workflow, Telstar reported that the licensing was still registered to one of their buyers and wasn¿t applied to the system. Telstar then had to re-register the server license with their company, download the licensing, and apply the license the City¿s database server to get the system back online. The system came back online and resumed data collection October 8th, 2020 at 2pm. The data was lost for October 6th and 7th, and cannot be recovered. City IT staff is currently working with Telstar to implement failover and notification options for the Ignition SCADA system, along with verifying server licensing for all SCADA servers. 1078916 07/06/2020 DMON The eSMR is missing a result for BOD, TSS and Hardness at location EFF-001B for July 6, 2020. The composite sampler at the site suffered a malfunction overnight leaving little volume that was not representative of the discharge. City operations staff will be conduction a comprehensive analysis of sampler conditions. City staff will then upgrade and replace samplers as needed. Completion of the analysis and samplers determined to need replacement will be dependent upon funds available. Violation U eSMR
1077487 06/05/2020 DMON June 5, 2020 the BOD and TSS sample at INF-001 was collected as a grab sample instead of a composite as required by the permit. The composite sample collected was inadvertently discarded in the laboratory as it was mistaken for a previously analyzed sample. 1077488 06/03/2020 Order Conditions June 3, 2020 the City experienced a turbidity reading over the 0.5 NTU maximum limitation at location FIL-001. It was found that the alarm to divert flow at 0.18 NTU failed due to a redundancy alarm setting within the system that caused a fault in programming. The redundant alarm setting has been removed at this time and this sort of program failure should not occur again. Violation U eSMR
1080283 04/30/2020 Order Conditions In the April 2020 eSMR cover letter, the City of Modesto reported that the effluent pH meter was put on hold by mistake from 14 April 2020 to 15 April 2020. Once the Facility¿s operators became aware of the mistake, the effluent pH meter was immediately put back to operation.
