On Sunday, June 14, 2026, a raw influent spill occurred at the Northern Regional Tertiary Treatment Plant (NRTTP) due to a PLC #1 failure and loss of SCADA/PLC control. The issue resulted in flooding at the headworks area and required manual operation of plant equipment, spill containment, agency notification, maintenance troubleshooting, and cleanup response. The spill occurred within the wastewater treatment facility headworks area and did not leave the facility. The overflow was routed through the plant¿s drainage and containment areas, including the stormwater basin and holding pond. The spill did not reach surface waters, did not enter a storm drain system that exits the facility toward the ocean, rivers, streams or lakes and did
not impact a drinking water supply. The total spill volume was 110,000 gallons, of which 66,295 gallons was recovered. |
Corrective action included maintenance troubleshooting and resolution of the PLC
issue, which returned the plant to normal operations. As an additional preventive measure, all UPS batteries were replaced, and ControlNet auto synchronization were performed on both PLCs and redundant modules. In addition, operators will be installing a complete secondary fixed float standby system as a backup control in
the event of a future SCADA/PLC system failure. The secondary float system will provide a hardwire mechanical float control to help prevent a similar overflow if the PLC system does not respond as intended. Additional review will be conducted to evaluate PLC redundancy, SCADA alarm response procedures, and backup controls to reduce the likelihood of a similar event occurring in the future. Staff response procedures will also be reviewed, and appropriate follow-up action, including retraining and/or corrective personnel action if warranted, will be taken to address the mishap and reinforce proper response protocols |

The lab failed HT for TSS sample collected on 10/4/2022. The laboratory also analyzed this TSS sample after the expiration of their ELAP certification. This was due to a lab staffing issue after the laboratory was bought out at the beginning of September 2022. |
The Laboratory immediately began to distribute the incoming samples to different sub labs beginning on 10/06/2022. See corrective action attachment. |
