1153774 03/04/2026 DMON Due to a communication lapse between the HSC personnel responsible for monitoring the flow meter at M-014 and the designated sampler, the sampler was unaware that flow had occurred via M-014 on 3/4, and 3/25-3/31/26. Therefore, the sampler was unaware that discharge to the CSD pond may have occurred during the month of March, and samples were not collected for laboratory analysis of total chromium and total zinc at monitoring location M-014, or for TSS and O&G at monitoring location M-017. The sampler checked for discharge at location M-017 on 3/27/26, and no flow was observed. It is unknown if the low flow observed at M-014 resulted in a discharge to the CSD pond during the month of March. The monitoring requirements of the permit have been clarified with HSC personnel. 1116964 04/19/2023 CAT1 Total Suspended Solids (TSS) Monthly Average limit is 30 mg/L and reported value was 32 mg/L at M-017. Persistent elevated TSS over the last roughly half year is thought to be a result of higher TSS in raw water used for the cooling towers. A bag filter was installed into the discharge line prior to the mixing tank in May 2023. Violation B eSMR
1113924 01/03/2023 CAT1 Total Suspended Solids (TSS) Monthly Average (Mean) limit is 30 mg/L and reported value was 49.3 mg/L at M-017. HSC will continue to monitor TSS; it is speculated that following high volume rainfall in January 2023, a higher than normal sediment load may have existed in the Eel River, which supplies the raw water used in our cooling towers. If the issue persists we will look at additional corrective action. Violation B eSMR
1093887 08/22/2021 PRE Following plant re-start, daily discharge rate on 8/22/21 at boiler blowdown (M015A) prior to mixing tank was 0.014 MGD, which exceeded the permit daily discharge maximum at that location of 0.011 MGD. However, total discharge to effluent discharge point (M017) was well below allowable limit. Startup often has higher than usual discharges; the boiler blowdown (M015A) exceeded the daily discharge limit of 0.011 MGD following restart on 8/22/21. However, it is a pretreatment location and the combined discharge at M017 (pond) on was 0.014 MGD (permit allows 0.048 MGD at M017) because there was no discharge from the cooling towers (M014).
