3. The plant was running for a short period in the early morning, from approximately 12:00 a.m. to 3:00 a.m. After completing our weekly maintenance cleaning, we were unable to get the plant back online. We were also occupied with installing a flow meter, which prevented us from getting the plant running in time for the lab to collect a sample that day. |
3. We had several callouts that day, which left us short-staffed. In the future, we will plan additional work more carefully and schedule it for days when staffing levels are sufficient. |

1. An operator failed to rotate the alum tanks when the level got low, despite signing off that the task had been completed. As a result, the plant went into a rolling count. The plant continued operating during the first part of the day until we were able to collect a sample and our 550 gate closed. We did have a sample from the previous 24 hours, but it was not collected on the same day as the incident. |
1. We spoke with the operator about the importance of accurately signing off on completed tasks and ensuring that work is actually performed before documenting it. This is to help prevent similar incidents from happening again. We also had our instrumentation technician investigate why the alarm did not call out sooner. |
