1. FOG Inspections by the Long Beach Health Department

Target Completion Date: August 13, 2025
The FOG Inspections by the Long Beach Health department were completed on schedule. (8/13/2025). The FOG inspections revealed that no violations were found among local businesses.
Planned Actions: Conduct inspections of nearby restaurants to identify issues related to fats, oils, and grease (FOG).
Collaborate with the operations manager to ensure regular cleaning of grease interceptors and proper FOG management practices are in place.
Utilize CCTV inspection equipment to trace the source of FOG buildup within the system.
Note: The area is televised and cleaned annually due to the Long Beach Grand Prix, the most recent CCTV was in February 2025 and the line was clear and open. |

Repaired Facilities or Replaced Defect. Upon arriving on-site, our team began corrective actions to stop the leakage and prevent future occurrences. Our approach consisted of the following steps:
1. Immediate Spill Stoppage: We replaced the corroded part under the bridge, effectively halting the leakage and preventing further sewage spillage into the water.
2. Investigation and Assessment: We conducted a thorough examination of the part and the surrounding system components to identify any additional areas of concern.
3. Devising Long-term Solutions: We will work with engineering to design a more robust system for the defective part on the air relief valve to withstand the harsh environmental conditions under the bridge, considering factors like moisture, saltwater, and potential physical impacts.
4. Regular Inspections and Maintenance: We will increase the frequency of inspections for this specific location and implement preventative maintenance procedures, as well as maintenance on the ARV. |

Adjusted Schedule/Method of Preventive Maintenance, Inspected Sewer Using CCTV to Determine Cause, Other (specify below). 1. The mainline segment was thoroughly hydro-jetted.
2. The preventive maintenance schedule was adjusted. The mainline segment has been added to our hot spot list for more frequent cleaning.
3. A Smart Cover Electronic Device will be installed under a City Sewer Manhole. This device monitors water flow and sends an alarm to LBUD when significant changes occur, serving as a warning of an impending spill.

Enforcement action against Fats, Oil, and Grease (FOG) source, Inspected Sewer Using CCTV to Determine Cause. The following major milestones have been identified for the corrective action plan:
-FOG Inspections by the Long Beach Health Department
- Target Start Date:10/31/2023
- Actions: Perform inspections on nearby restaurants to identify FOG-related concerns.
- Addressing FOG Concerns at PCH & 2nd Outdoor Mall
- Target Completion Date: 11/1/2023
- Actions: Collaborate with the operations manager to ensure regular cleaning of interceptors and proper management of FOG. Utilize CCTV inspection equipment to identify the source of FOG and conduct more frequent inspections.
- Emergency repair due to possible cross bore in the sewer mainline not caused by CS.
- Target Completion Date: 11/30/23
-Actions: Monitor and clean the main until repairs are complete
