1148742 10/17/2025 SSOS Type: Category 1 Spill; Debris-General caused 10 gallons of sewage to spill from Manhole at 6 Morningside dr. San Anselmo to Street/Curb and Gutter (2 3),Surface Water Post Spill CCTV,Other (specify below). CCTV inspection on 10/20/25 did not reveal any debris or defective pipe segments. The following day 10/21/2025 the line was hydro flushed and moderate debris was observed from the syphon. The line/syphon is scheduled for 6 month flushing intervals. Violation A SSO
1148747 10/13/2025 SSOS Type: Category 1 Spill; Pipe Structural Problem/Failure - Controls,Root Intrusion,Fats, Oil and Grease (FOG) caused 8220 gallons of sewage to spill from Manhole at 965 Magnolia Ave, Larkspur to Surface Water Post Spill CCTV,Other (specify below),Inspected Sewer Using CCTV to Determine Cause,Repaired Facilities or Replaced Defect. A post spill CCTV inspection revealed a large root mass still present at an offset joint in the 6" clay pipe main. Additionally, a section of the sewer main was replaced with a cast iron section, and the cast iron section has dropped out and is missing pieces on top. Staff hydro jetted the line and removed the root mass. In house repair crew has scheduled a repair for both the offset joint with the root mass and the cast iron section of pipe. Engineering is also reviewing this section of pipe along with upstream sections to determine if full replacement and upsizing make sense. Violation A SSO
1145936 08/03/2025 SSOS Type: Category 1 Spill; Collection System Maintenance Failure (specify below) caused 197 gallons of sewage to spill from Manhole at 14 Terrace Ave, Greenbrae, CA to Drainage Conveyance System that discharges to surface water Other (specify below),Adjusted Schedule/Method of Preventive Maintenance,Inspected Sewer Using CCTV to Determine Cause. Corrective action includes an additional box added to the work order, to ensure all tools/equipment have been removed from the MH. Currently the District asset management team is working to incorporate the new information box into the workorders. The expected implementation for this is the end of August 2025. Violation A SSO

The spill was caused by a crack in the VCP main. The defect was fixed with a cured in place fiberglass patch. This portion of the main happens to be in the flow of the stormwater conveyance system. Operations staff is recommending the engineering department look at moving main out of the stormwater flow or upgrading piper material. Violation B SSO

The District has scheduled maintenance repairs in the up stream manhole to improve the flow channel.
