1126521 03/23/2024 CAT2 Chlorine, Total Residual 1-Hour Average (Mean) limit is 0.019 mg/L and reported value was 1.245 mg/L at EFF-001A. On 3-23-2024 the chlorine residual exceeded our permit limit while the bisulfite pump was not running, meaning there was no bisulfite to counter act the chlorine residual. This happened between 1034 and lasted until 1045, or approximately 11 minutes. The average flow for this time frame was 0.426 MGD which means approximately 3256 gallons of water was sent to the Effluent Pond without de-chlorination. The Effluent Pond is 410¿ x 130¿ and the depth at the time was about 2.6 ft. meaning it would take about 58 hours for the chlorinated water to reach the river discharge structure. This violation was discovered on 4-3-2024. An email notification to the RWQCB was sent that same day. Violation U eSMR
1125324 02/20/2024 OEV Total Coliform Daily Maximum limit is 240 MPN/100 mL and reported value was 350 MPN/100 mL at EFF-001T. The coliform sample was collected at a time where the chlorine analyzer was experiencing issues and it is likely that not enough chlorine residual was present. Operators were alerted of the issue and we don¿t expect to experience this in the future. Violation U eSMR
1125325 02/04/2024 OEV Total Coliform Daily Maximum limit is 240 MPN/100 mL and reported value was 540 MPN/100 mL at EFF-001S. The coliform sample was collected during a surge in flow and we suspect that the disinfection system had not stabilized. Operators were alerted of the issue and we don¿t expect to experience this in the future. Violation U eSMR
1116007 03/14/2023 OEV Total Coliform 5-Sample Median limit is 23 MPN/100 mL and reported value was 33 MPN/100 mL at EFF-001S. It's not clear what caused the relatively high Total Coliform results over the six days with EFF-001S discharges this month. Total Coliform dropped significantly once high flows subsided and the filters were started again in the second half of the month. Violation U eSMR

We don¿t foresee this being an issue moving forward; a sample was taken in January, as required, and results are pending. E. coli sampling is required weekly, when discharging to the river, at both EFF-001S and EFF-001T. Flow was produced at EFF-001S over a period of two weeks, from 12/15 to 12/28, and E. coli samples were collected on 12/17 and 12/24. Unfortunately, the lab analyzed the 12/17 for total coliform, instead of E. coli as requested on the Chain of Custody (COC) form. As such there is no E. Coli result to report for 12/17 at EFF-001S. The 12/24 result was non-detect, below the analytical detection level of 1 MPN/ 100mL. Flow was produced at EFF-001T over a period of three weeks, from 12/1 to 12/14 and again from 12/29 to 12/31. E. coli samples were collected weekly on 12/17, 12/24, and 12/30, and the analysis requested on the respective COC forms. Unfortunately, the lab failed to analyze for E. coli on all three occasions. Moving forward, we have alerted the lab to this issue and they assured us that all required analyses will be processed in the future. Copper sampling is required monthly when discharging to the river, but unfortunately staff did not request this analysis from the analytical laboratory.
