By the time the mistake was caught the contract lab had disposed of the source. No TSS Value is available for the week of 11/07/24 for INF-001. |
More care will be taken in the future to evaluate laboratory results as soon as they arrive from the contract laboratory to catch issues like this with enough time for a sample to be analyzed before it is disposed. |

August 20th 2024: The Coliform sample collected for monitoring location EFF-002 (Discharge point 002) was analyzed using SM9223 instead of SM9221. The reason for the incorrect analysis was due to an error on the Chain of Custody which requested 9223 instead of 9221. The chain of Custody format has been updated to reduce the chances of recurrence.
| September 17th 2024: No Coliform sample was collected for sampling location REC-001 (Discharge point 003) even though discharge had occurred on that day. The sample was not able to be collected while producing Recycled Water due to a communication failure caused by upgrades being performed on the fiber optic communication system.

EFF-002(8/20/24): Chain of custody documentation has been revised to reduce the potential of this mistake occurring in the future.
REC-001(9/17/24): This error was caused during upgrades to the system and was not anticipated to occur, nor is it expected to return. |

March 11, 2024 - Between 438,000 and 525,000 gallons of Secondary Treated Wastewater was discharged to the Russian River through Discharge Point 001 (EFF-001B) due to an operator error involving the set point conditions of several valves. |
The City is planning to revise its River Discharge SOPs to require supervision from a Senior Operator or higher when isolation valves are opened/closed and to include a 3-D diagram of the valve system to increase staff understanding of all connections and potential impacts. Refresher training on river discharge procedures will be held for all operations staff each year, approximately one month prior to the start of the river discharge season. In addition, two auto actuation valves will be installed as part of the Phase 4 Recycled Water Project. This installation will provide automatic closure of the River Discharge Isolation Valve when it is not being used and when off spec water is produced. The project has been designed, funded, and is currently underway. However, the lead time for receiving the valves is 48 weeks so installation will not occur until the summer of 2025 |
