1102864 03/01/2022 UAUTHDISC Jacket Water/Closed Loop System unauthorized release. A valve on the system that is automated has been broken so the valve has been manually operated (opened and closed). Wind blow debris (cardboard) got trapped onto a drain because of the suction caused by the drain in the basin. That seems to have resulted in overflow or release of closed loop system water to the ground/paved surface surrounding the drain basin. 20 to 40 gallons. The water has evaporated or infiltrated into the soil areas around the compressor building. A sample was collected of the water. Violation of WDRs, Section II, 9. Water released evaporated. Water analytical results received March 30, 2022. Valve to be repaired by March 11, 2022. Violation B Report
1102418 12/31/2021 DMON Failure to maintain in good working order any control system or monitoring device installed to achieve compliance with WDRs. Multiple lysimeters not functioning properly. P8-LY-2E - not functional since May 2016, P8-LY-5W - not functional since July 2018, P8-LY-1E - not functional since October 2019, P8-LY-3E - not functional since second half 2021. 1102419 12/31/2021 DMON In October 2020 PG&E implemented remedial actions for chromium cleanup east of the surface impoundments. Those actions resulted in a change in groundwater flow direction from the north, north-west to the east. The Point of Compliance, therefore, was changed. PG&E failed to add monitoring points to adequately monitor the changed Point of Compliance. Violation of WDRs III.H.1., MRP II. On March 11, 2022, PG&E submitted documentation identifying potential monitoring wells east of the surface impoundments and adequate to establish a new Point of Compliance. Violation B Report
