1112134 10/11/2022 DMON The October Effluent sample was received in good condition at the BSK San Bernardino satellite laboratory on 10/11/2022 and logged in by BSK-RAL for Cyanide testing. The sample was then shipped overnight to the BSK Fresno main laboratory. However, a Facility Code in the transit chain-of-custody was inadvertently left out for the Cyanide testing, thus failing to designate the Cyanide test to be performed by BSK-FAL, instead of BSK-RAL. Consequently, even though the sample arrived at BSK-FAL on 10/12/2022, and the Laboratory Information System (LIMS) had a Cyanide test logged in for the sample, the test did not show up amongst the worklists to be performed by the Wet Chemistry department at BSK-FAL. By the time this login error was discovered, the sample container was already disposed and the sample had already passed the 14 days holding time for Cyanide testing. When BSK notified us, it was too late to resample. There were no reported results for Cyanide in the month of October. BSK apologized for missing the Cyanide testing for the 10/11/2022 Effluent sample and that it was discovered too late for the City of Simi Valley to resample to meet our monthly monitoring requirements. BSK clearly stated that the missing Cyanide result was due to the laboratory's login error, and not the fault of City of Simi Valley. BSK has created a new LIMS query for any items that are missing Facility Code in the transit COC and will be re-training all the staff with the proper transit COC procedures. They believe the corrective actions will adequately address this deficiency and prevent the error from happening again. The Simi Valley Water Quality Control Plant Laboratory staff were also instructed to check that all samples have been received by the contract lab and are in process before the end of each month. The suspected cause of the exceedance is due to the ongoing construction at the treatment facility. Some of the construction requires operations to remove one of our seven aeration basins from service resulting in less than full treatment capacity. To help remedy the situation, we have been adjusting process controls such as RAS/WAS rates and addition of polymers to the process to help with settling. Violation B eSMR

The plant also continues to limit waste hauler dumping which has resulted in the improvement of toxicity in recent tests. During this process, the SVWQCP continues TRE/TIE activities while continuing the monthly compliance monitoring.
