On November 27th, the City was notified by the sublab, that the WRF No. 1 BOD influent sample
collected on November 18th was reanalyzed past hold time due to failure of quality control parameters in the initial analysis. |
The root cause was due to a drop off in the PolySeed performance over the 14-hour work day. The sublab found refrigeration of the PolySeed with removal of aliquots throughout the day for setting up of additional batches has improved the vitality of the seed. The lab will continue this path with samples being run in tandem using the natural seed until they are certain that it is sustainable. A corrective action report was requested from the lab and is attached to this report. |

On October 16th, the City was notified by the laboratory that the Lab Control Sample had low bias readings for WRF No. 1 BOD influent sample collected on 10/7/24 due to instrument malfunction/stirrer on DO probe not stirring.
On October 23rd, the City was notified by the laboratory that the Lab Control Sample failed for WRF No. 1 BOD influent sample collected on 10/14/24 due to low seed strength. |
On October 16th, the City was notified by the laboratory that the Lab Control Sample had low bias
readings for WRF No. 1 BOD influent sample collected on 10/7/24. The lab conducted an investigation
and found the stirrer on the DO probe was not spinning, a malfunction staff was unable to troubleshoot.
The final BOD reading was made without stirring, which resulted in the bias-low readings. The WRF
No. 1 BOD influent sample was reanalyzed past hold time and was within historical range. A corrective
action report is included in this report.
On October 23rd, the City was notified by the laboratory that the Lab Control Sample failed for WRF
No. 1 BOD influent sample collected on 10/14/24 due to low seed strength. As noted in the previous
corrective action report, the lab had successful trials with PolySeed with the use of bottled spring
water, however, the water was ¿too clean¿ to sustain the biological activity of PolySeed. The lab is
currently vetting the use of tap water and has undergone successful rounds. The natural seed remained
in use and has started to fail acceptance criteria in the third week of use. Delivery was updated from
every three weeks to every two weeks and has been passing viability testing. The WRF No. 1 BOD
influent sample was reanalyzed past hold time and was within historical range. A corrective action
report is included in this report. |

On July 12, 2024, the City was notified by the contract laboratory, Eurofins, that the CalScience sub-laboratory had been experiencing contamination issues with one of their BOD instruments and have had failing Unseeded Control Blank and Laboratory Control Samples. They have been working on mitigating the issue by ordering new parts to replace instrument tubing and to conduct decontamination cleanups. Samples impacted include WRF No. 1 July 3rd weekly effluent BOD sample and the July 8th weekly BOD influent sample. Impacted samples with results that did not align with historical values were reanalyzed past hold time for result confirmation. A corrective action report was requested for these samples and is included in this report. |
The backup instrument was prepared for the next batches and troubleshooting the contamination began. The plan was to replace all tubing and carboys. Most were available in stock and some had to be ordered. The bottle cap cleaning regimen was changed to using bleach and oven drying. Once the ordered tubing had been received and all tubing was replaced, a blank study was performed. The study took the entire five days. The results were still showing detections in the USB. There was a small length of tubing that was used to extend the seed tube dispenser that had been overlooked. This tube was removed and was not replaced. Another blank study was performed. This time the blank was clean.
Corrective Action:
1. Bottle cap cleaning regimen was changed to using bleach and oven drying.
2. Replacement of tubing on a schedule.
Contamination of the BOD instrument system is a common issue in this analysis. The 5-day closed system leaves the outcome uncertain. We make best efforts to sustain a clean system with the unclean samples we expose it to. An additional blank study was begun on 8/19/2024 as an additional quality control measure. We expect that study to return clean and the instrument can be put back on line.

On May 8, 2024, the City was notified by the contract laboratory (Eurofins) that the sample for Bis (2-Ethylhexyl) Phthalate was lost by the sub lab, CalScience, during the concentration phase and re-extraction was performed past hold time. Notification was received after the monitoring period, therefore, a resample was not collected. |
The root cause of the issue was due to a lack of immediate notification from the sub lab to Project Managers when issues arise during extraction process. Corrective actions from CalScience include Operations initiating continuous training sessions with the Extractions Team Leads on sending Non-Conformance Memos promptly to Project Managers, City of Corona will be added to the daily emails that follows specific jobs through the laboratory to ensure they are completed on time, and another possible strategy would be to split the Calscience and/or other subcontract work off into a job 2, 3, etc. on log in. Job 2 will go to job complete quicker than the remainder of the work and could be checked before the job due date, which would double check any missing work from subcontract labs. |

On January 16, 2024, the City of Corona was notified by the contract laboratory, Eurofins, that the WRF No. 1 total coliform sample, collected on January 10th, did not meet the 48-hour incubation requirement due to laboratory error. During the transfer to the confirmation phase, the transferred samples were not incubated, rendering unreportable results. This was an error by the analyst, where they switched the prior day's completed samples with the January 10th samples by accident. |
The City requested a Corrective Action Report for this incident and is included in this report: As a means of prevention of recurrence of this issue, the laboratory is using color tape to distinguish each day of the week of analysis, thereby creating a very visible means to identify potential errors.
The noted corrective action plan should prevent a recurrence of such an issue. |

The cause of the high turbidity was due to hi-solids inventory overload in the system. Operations staff closely monitored turbidity levels and were able to maintain compliance with no additional turbidity exceedances. The RWQCB was immediately notified on the day of the violation. |
