R8 Annual Compliance Summary was submitted on the April 1, 2022 due date, as required. After submittal, it was discovered by IRWD staff that the Appendix N did not include a report component. Appendix N did not include the Total Nitrogen and Total Phosphorus Offset Plan Calculations documents. |
IRWD resubmitted the Annual Compliance Summary report which included an updated Cover Letter explaining that IRWD identified a missing component and an updated Appendix N to include the missing Total Nitrogen and Total Phosphorus Offset Plan Calculations. The original report was submitted as part of the March monthly WDR compliance report and resubmitted with the April monthly WDR compliance report.
A submittal checklist was created to prevent this incident from occurring in future submittals.

An equipment malfunction with the aquatic monitoring vehicle resulted in the scheduled monthly samples not being collected at Rattlesnake Reservoir (R-001) and therefore they needed to be rescheduled. Due to a miscommunication between monitoring staff on scheduling, the sampling event was not rescheduled and the monthly samples were not collected. |
Staff determined the corrective action was to retrain all monitoring staff on the existing sample scheduling processes and specifically identified an additional staff member who will be trained by a supervisor and dedicated to reviewing the sampling schedule assignments to ensure all scheduled monitoring activities are completed. This will provide a secondary level of review for added redundancy and the assurance that the required sampling is captured. |
