Electricians arrived on site to reset Lift Pump #2 and influent flows increased to more than 43 MGD; Lift Pump #1 was not able to be restored during this storm. 1128649 02/04/2024 Order Conditions Did not comply with wet weather plant operational requirements, as described in Provision VI.C.5.c.iv. INF flow was not maximized due to a power outage at the Westside Pump Station, which stopped all pumping to OSP. Per the February SMR cover letter, CSDs from Vicente and Lincoln occurred on 2/4 at 7:15 p.m. and 7:40 p.m., respectively. It is not possible to determine whether CSDs would have occurred if there hadn’t been a power loss. SFPUC staff used a backup generator to run a smaller pump until power was restored. 1124422 11/29/2023 Order Conditions Did not comply with wet weather plant operational requirements, as described in Provision VI.C.5.c.iv(b). INF flow was not maximized while discharge from the Westside Transport/Storage Structure to Discharge Point No. 001 occurred. 1124424 11/28/2023 UAUTHDISC A dry weather discharge occurred on November 28 from CSD-007 (Sea Cliff No. 2 pump station) between 10:09-10:16 a.m. An estimated 5,500 gallons of untreated wastewater was discharged. Per SMR cover letter: "Corrective action is an ongoing progressive disciplinary process for the certified operators on-site when discharge occurred which will result in grounds for action up to and including termination. Additionally, lessons learned have been included for any planned process related projects in the future." Violation B Report

1091449 02/15/2021 Order Conditions Did not comply with wet weather plant operational requirements, as described in Provision VI.C.5.c.iv(b). INF-001 did not reach 60 MGD prior to initiating discharge from the Westside Transport/Storage Structure to Discharge Point No. 001. This resulted in approximately 0.51 MGD (6% of total decant for the day) of extra untreated wastewater being discharged through EFF-001. Decant discharge pumping occurred early due to a misreading of the OSP influent flow meter. Wastewater Operations staff have been reminded of this minimum flow value and a large sign with the information has been posted in the plant control room. Violation B Report
