1123723 10/17/2023 CTOX Chronic Toxicity Other limit is 0 Pass/Fail (Pass = 0, Fail = 1) and reported value was 1 Pass/Fail (Pass = 0, Fail = 1) at EFF-001. Follow up testing was not conducted and no corrective action was taken. 3 days after this sample was collected and delivered to the lab the treatment plant effluent was shut down and ponds were diverted to an emergency basin to investigate a potential liner leak. Unofficial results for this sample were received October 23rd, days after the plant stopped discharging. Violation U eSMR
1122302 10/11/2023 DMON Seeley CWD conducted sampling for effluent Cyanide per monthly requirements. Samples were stored appropriately and delivered to laboratory who then failed to test samples within holding times. Resampling for the monthly sample was no longer an option as the facility was now offline and no longer discharging to receiving waters. This is an error on the part of the contracted laboratory and there are no corrective actions to be taken by District staff except to start looking at alternatives to this laboratory, if possible. 1099866 10/07/2021 Deficient Reporting Seeley CWD staff collected effluent samples for Ammonia analysis. Samples were delivered to satellite location with chain of custody. Sample was damaged en route to main laboratory location and due to a miscommunication the effluent Ammonia sample analysis was cancelled and this information was not reported by the lab to Seeley staff. By the time this issue was discovered any remaining sample was past holding times and the quarter had ended. Attached is a letter from the laboratory's quality assurance director explaining the situation. We cannot comment on what actions the laboratory will take in the event of a similar event happening again. However, Seeley staff will promptly review laboratory results as they arrive for any missing results. Violation U eSMR

Our treatment plant is working on a compliance project specifically related to effluent copper and cyanide concentrations. Additionally we are compiling receiving station users by gallons dumped to see which companies were new in March that may have contributed or caused the effluent cyanide violation. If necessary and this problem persists those users may be barred from using our facilities. Violation B eSMR
1089335 03/10/2021 CAT1 Biochemical Oxygen Demand (BOD) (5-day @ 20 Deg. C) Weekly Average (Mean) limit is 65 mg/L and reported value was 82 mg/L at EFF-001. There was an increase of dump station/receiving station traffic in March. However, all valves within the treatment plant were checked to eliminate any possibility of short circuiting; as increased load in the plant influent would take time to show up in effluent. Additionally, daily checklists and log book entries were reviewed to find causes. Effluent BOD sample for the following weeks up to the date of this writing are back within acceptable limits (less than 20mg/l). Violation B eSMR

1066674 11/02/2019 Deficient Reporting Although we did collect and submit effluent copper, cyanide, mercury, and bis(2-ethylhexyl)phthalate samples to the local laboratory we have yet to receive those results from the lab despite numerous attempts at collecting those reports. Attached to this report are the chain of custody sheets showing collection date/time as well as received by information for lab personnel. 1: If the local lab submits these results we will request from waterboards to have this report withdrawn so we can resubmit with the lab results. 2: As of November 2019 we began using Babcock laboratories for the majority of our wastewater sampling because of the issues we've had with the local labs. 1070291 10/08/2019 Deficient Reporting To date lab has failed to report back to us various results. Samples were collected and delivered to the lab. We have reached out to the lab which has since closed its door leaving us with a number of pending results. We will still continue to request results to the best of our ability and we will request to have this report withdrawn once we receive the pending results so we can submit the complete report. Week prior to this violation effluent BOD was within normal acceptable levels, and the 4 weeks following this violation effluent BOD was also within normal/acceptable levels. There is nothing in our log books or daily checklist that indicates that anything was out of the ordinary. Violation B eSMR
1063455 07/01/2019 DMON Copper and Cyanide sample was not shipped out by laboratory to subcontracted laboratory meaning that there are no Copper and Cyanide results to report. Working on switching labs. Violation B eSMR
