1099707 08/17/2021 DMON SCRWA LAB)The BOD test standard did not meet the QC requirements. The GGA was a 113 mg/L, the Secondary Effluent, Influent and the Reclamation samples were in the normal range. The reading for the blanks was a 0.1 mg/L and the seed depletion was 0.5 mg/L. Possible error with the seed. SOP updated to include a system for ensuring proper volume of seed measurement occurred Violation B eSMR
1099706 07/27/2021 Order Conditions A filter feed pump failed but did not call out for the alarm. The level dropped in the CCB causing the Detention time to drop below 90 minutes for 70 minutes. Pump was taken out of service and checks were performed on the alarm system to ensure it is functioning properly. Violation B eSMR

1085147 11/10/2020 Order Conditions The contact time for chlorine contact basin dropped below 450mg-min/L for 11 minutes. The on-call operator received an alarm for low effluent chlorine residual, and they made an adjustment. The alarm was latched preventing a second call out when the contact time started to drop. (limit 450mg-min/L alarm setpoint 460mg-min/L) We are working with our SCADA techs to resolve the issue of SCADA not calling out if an alarm is latched in the system. We are retraining staff on proper procedure when addressing alarms from home to ensure alarms are not missed. Violation B eSMR

1085148 06/06/2020 DMON Contract Laboratory missed the standard and the hold time on a Reclamation Effluent Sulfide sample. Data will be disqualified and not included in the averages. We are collecting the sample later in the day. This will give the contract lab extra time to analyze the sample. We are in the process of getting certified for this parameter. 1070107 11/19/2019 DMON November 19, 2019, it was discovered that the temperature in the lab was much warmer than normal. This excess heat caused the coliform incubator temperature to climb to 39.2 degrees Celsius. This event affected the samples collected on November 17th, and November 18th, 2019. We immediately shutdown the unit and contacted the vendor who is updating our HVAC system. When the new HVAC system was installed the contractor was having issues getting the old controllers to communicate properly. We are in the process of having the controllers upgraded. The HVAC contractor is currently working on installing the new controllers. Violation B eSMR
1070108 09/25/2019 DMON September 25, 2019, it was discovered that the temperature in the coliform incubator room was much warmer than normal. This excess heat caused the coliform incubator temperature to climb to 40.1 degrees Celsius. This event affected the samples collected on September 23, 2019 and September 24, 2019. The HVAC has been installed and is online. Compliance samples collected on September 25, 2019 were sent to an outside lab as a precaution if the HVAC was not completed on September 25, 2019. 1070109 09/15/2019 DMON September 15, 2019 it was discovered that the door to the coliform incubator was not fully closed causing the temperature to drop to 34 degrees Celsius. This event affected the samples collected on September 13, 2019 and September 14, 2019. The door was latched, and the Lab Supervisor was notified. The samples were still analyzed. Neither had any confirms. Going forward the incubator door will not be opened unless to check temperature for compliance reporting. On the weekends two operators will do a final walk through of the lab to ensure the proper state of the lab before leaving for the day. Violation B eSMR
