Additional training has been provided to operations staff on surge mitigation during filter transitions. |

A filter feed pump failed but did not call out for the alarm. The level dropped in the CCB causing the Detention time to drop below 90 minutes for 70 minutes. |
Pump was taken out of service and checks were performed on the alarm system to ensure it is functioning properly. |

Order Conditions |
The contact time for chlorine contact basin dropped below 450mg-min/L for 11 minutes. The on-call operator received an alarm for low effluent chlorine residual, and they made an adjustment. The alarm was latched preventing a second call out when the contact time started to drop.

We are working with our SCADA techs to resolve the issue of SCADA not calling out if an alarm is latched in the system. We are retraining staff on proper procedure when addressing alarms from home to ensure alarms are not missed. |
