The City of San Clemente, M-INFD, did set up an Influent cBOD that week of 7/19 but the tech was out the following week due to an accident and was unable to complete the analysis. The tech normally setup Influent cBOD twice a week to ensure that they have at least one data point but he happened to be out that Thursday which is the day the tech normally setup the additional Influent cBOD. |
Moving forward, City of San Clemente will have the Operators send out an Influent Composite sample for cBOD at least twice a week when there is no lab staff available. |

The City of San Clemente, 001D, Water Quality Laboratory sent out the Effluent Composite sample on June 17, 2025 to Sierra Analytical and failed to list Total Suspended Solids on the list of analyses requested. |
Moving forward, the laboratory will routinely sample and report TSS 6 days a week to ensure we have at least 5 days of sampling in the event one is missed. |

At CSC M-001D, Orange Coast Analytical, a contract lab, failed to analyze for cBOD on 12/20/24 even though it was clearly indicated on the chain of custody. |
Because of their inconsistencies, we will not be subcontracting out to Orange Coast Analytical anymore. Our usual commercial lab, Sierra Analytical, has recently renewed their CA-ELAP certification and they will be setting up cBOD samples from now on. |

We (City of San Clemente M-001D) have no Total Suspended Solids data for the Effluent Composite sample on November 22nd. The subcontract laboratory failed to run TSS even though it was listed on the chain of custody. We notified the contract lab of the error. |
In response to the error made by the sub-contracted laboratory in question, we have inquired about alternative laboratories to contract in case of these types of errors continue. We have also discussed the error with our normally contracted lab to bring this matter to their attention. We will continue to closely monitor all lab analyses done by outside labs to prevent this from happening in the future. |

On November 3, 2021 a laboratory error occurred in setting up the cBOD for the Effluent sample for M-001D. Nitrification inhibitor was not added to the cBOD bottles, essentially setting up BOD instead of cBOD. |
1) All cBOD bottles will be double checked for nitrification inhibitor prior to dilution water being added to the bottle.
2) Effluent cBOD will now be analyzed Monday-Saturday (6 days) insuring that the City of San Clemente will have at least 5 days of reportable cBOD data.

CSC 001D We missed an Effluent Composite sample for the week of 11/29-12/05. Analytes missed were TSS and cBOD. These analytes are to be sampled 5-days per week. The Effluent composite sampler was left in Grab sample mode over the weekend. Therefore, there was not composite sample on Monday, November 30th. The sample was rescheduled for Saturday, December 5th. Although I had scheduled the sample in Lab Cal, I simply forgot to collect it since it was outside of our normal sample routine. |
-Will retrain operators helping in the lab to make sure that the composite sampler is setup correctly at the end of each day.-Will alert Operations Supervisor immediately whenever a sample is missed to provide an extra layer of accountability.
-Will put an alarm in Microsoft Outlook to remind me of samples that are outside our normal schedule. |
