For the week of August 18th, the weekly bacteriological sample at EFF-001 was missed due to limited resources at the time. The seawater well and Desalination plant had been offline for a period of 8 months for upgrades. During the recommissioning/startup of the seawater wells and Desalination Plant, it required the operators to perform a combination of new processes and troubleshooting to ensure the plant was working properly. This caused a strain on resources, and the samples were missed for the week. |
Resources will be evaluated in the future after long-term outages to ensure all operational goals and compliance requirements are met. |

The July monthly sample for EFF-001 was inadvertently missed due to an oversight. The samples were marked as taken but in essence they were not taken during the period of July. Additionally, the third quarter 2024 salinity samples were not taken for the three locations at the three depths. This was an administrative oversight. |
New safeguards are in place to prevent missed samples in the future. A new checklist for all required samples has been created; the checklist will be checked off twice:
When the Chain of Custody and samples are sent off to the contracted laboratory;
When the Chain of Custody is acknowledged by the contracted laboratory and reported on their database. |

For 001-Y in Dec. 2020, only PCBs (as Aroclors) were analyzed, and PCBs (as Congeners) were not. The violation was noted in the cover letter, but there was no mention of a plan to sample or to return to compliance. |
Andrew Choi spoke to Paul Ahn from So. Cal Edision on 5/21/21. Paul informed that the chain of custody template was modified to indicate to test for both Aroclors and Congeners. So. Cal Edison staff were also trained to make sure that both Aroclors and Congeners to be sampled and tested. Their lab has been notified of this issue and the modifications on the chain of custody to test for both Aroclors and Congeners. |
