The ship then notified the base fuel response team of the release which led to Port Operations to come investigate the vessel for any indication of what caused the release. It is speculated to be a release of natural causes unrelated to the vessel and thus was cleared for continued operation. BOOMs and absorbent pads were applied, but the sheen was out of their reach and the release is deemed unrecoverable and expected to dissipate naturally. |

Per the NBSD permit, a toxicity reduction evaluation (TRE) was not required.
Additionally, evaluation of best management practices (BMPs) at Pier 4 was conducted to identify contaminants of concern in an effort to reduce toxicity in storm water runoff and to facilitate modifications to the Storm Water Pollution Prevention Plan as necessary. |

An investigation identified a pin-hole in the top portion of the fuel tank and the side of the hull due to corrosion, so when the tank was filled, diesel fuel was released from the tank. |
The hole in the tank was patched with a temporary patching material to stop the spill, then spilled fuel was recovered with absorbent pads. The fuel tank was de-fueled on the morning of 6/14/2021, so that the tank can be repaired permanently or replaced. |

An investigation to the cause of the exceedance did not identify a source. The Stormwater Discharge Visual Observation form completed by the MMEC sampling team did not note any visual evidence of pollutants at the time the sample was collected. Industrial High Risk Facility operators perform pre-rain inspections before storms and NBSD Environmental personnel inspect all piers at least weekly. No sources of contaminants or best management practices (BMPs) deficiencies were observed on Pier 4 before the storm event. Evaluation of the BMPs associated with Outfall 218-247 (NBSD-152/Pier 4) is now being conducted concurrently with and in addition to weekly pier inspections. Sources of contaminants of concern will be evaluated in an effort to reduce toxicity in storm water runoff and to facilitate modifications to the Storm Water Pollution Prevention Plan as necessary. NBSD will continue to monitor the situation for High Risk Industrial Storm Water Areas (NBSD Permit, Attachment E, Section V.B.). |

Although the NGD emergency fire suppression and salt water supply system has been repaired several times to prevent discharge to San Diego Bay under normal operating conditions, the component of the system that prevents discharge has failed shortly after each repair. NBSD continues to evaluate options to repair the system to operate without discharge to San Diego Bay. |

Evaluation of the best management practices (BMPs) associated with Outfall 218-247 (Pier 4) was conducted concurrently with and in addition to quarterly inspections. Sources of contaminants of concern were evaluated in an effort to reduce toxicity in storm water runoff and to facilitate modifications to the Storm Water Pollution Prevention Plan as necessary. NBSD will continue to monitor the situation for High Risk Industrial Storm Water Areas (NBSD Permit, Attachment E, Section V.B.). |
