1119689 03/13/2023 Surface Water Pier 13: Discharge of approximately 25 gallons of F-76 fuel onto the pier, storm drain, and San Diego Bay. Discharge was from an overflowed portable tank on the pier that was being used to drain piping that was being replaced on a ship. Approximately 20 gallons on the pier and approximately 5 gallons entered the storm drain. Contractor cleaned up the spill on the pier. Oily wastewater in the bay unrecoverable. Spill contained and kitty litter used to absorb spill. Contractor will review procedure to minimize risk of future overflow of portable tank. Violation U eSMR
1119688 01/26/2023 Surface Water Pier 7: Approximately 50 gallons of marine diesel (F76) was released from a head tank vent for a generator onboard the ship. The release occurred when opening a valve to shift standby generators. Approximately 30 gallons was released into San Diego Bay. The spill was contained within the ship's oil containment boom. 1119694 01/24/2023 Surface Water Quall Wall 4: .5 gallon of hydraulic oil (10W-30) was released from the starboard side propeller during maintenance on a seal. A boom was already in place around the cutter. The hydraulic oil is contained within the boom area. The FRT team was notified and responded to the spill, however, the spill was unrecoverable. 1119687 01/22/2023 Surface Water Pier 7: Fuel oil system had a mechanical error which led to the discharge of 30 gallons of fuel oil into the San Diego Bay. The release was stopped immediately and booms were deployed to contain fuel oil. The spill was contained within the ship's oil containment boom. The FRT spill response team responded and recovered the spill using absorbents. A ship's message was sent out. 1119677 01/05/2023 ATOX Acute Toxicity-Mysid, H. costata or M. bahia-survival Maximum Daily (MDEL) limit is 40 % effect and reported value was 41.7 % effect. A sample (verification test) was collected at outfall 218-247 (Pier 4) during the next QSE on March 1, 2023. The sample resulted in a 13.3% effect compared to the Salt Control, which failed the TST analysis. However, the percent effect was less than 20% and did not exceed MDEL threshold. Per the NBSD permit, a toxicity reduction evaluation (TRE) was not required. Additionally, evaluation of best management practices (BMPs) at Pier 4 was conducted to identify contaminants of concern in an effort to reduce toxicity in storm water runoff and to facilitate modifications to the Storm Water Pollution Prevention Plan as necessary. 1107675 06/13/2022 Surface Water A release at Pier 3 was reported of 2 gallons of sea water and an unknown oil into the storm drain in the maintenance laydown area. Ship reported contractor (SBSBTC) was blowing out a hose that had traces of the the unknown oil. 1107671 06/10/2022 Surface Water Pier 8: A vessel reported a spill of 1/2 gallon hydraulic oil from their vessel into San Diego bay due to operator error. This was caused by the use of a contaminated hose. Ship personnel and FRT responded to the spill however, due to the small volume the spill was unrecoverable. 1107664 06/01/2022 Surface Water Pier 13: An off-line station released bilge slops into the harbor at pier 13. The cause of the release was due to an equipment failure in the station. The amount of material that released is currently unknown, however the caller suspects 15-20 gallons may have released. The release occurred from the naval base not a vessel. The spill was contained within the ship and pier booms. The FRT spill response team and personnel from the BOWTS responded and recovered the spill using absorbents. The oily waste system operators performed a survey of the entire system and will review procedures and continue to monitor the system. Violation U eSMR

1107662 03/23/2022 Surface Water Pier 13: A vessel port fuel tank ruptured causing a spill in bilge space. In order to not lose access to the space and prevent further damage to ships equipment, it was necessary to discharge the fuel that was not contained in the bilge overboard. 4628 gallons discharged to San Diego bay. Unknown cause for the rupture of the fuel tank. Initial clean-up response was conducted by ship personnel and the NBSD Facility Response Team (FRT). A second oil containment boom was deployed around the ship and the fuel was recovered using absorbent materials and skimmers. The ruptured fuel tank will be repaired immediately. 1107655 02/28/2022 Surface Water Pier 8: A vessel reported the vessel activated the AFFF firefighting system without the SOPV (SOLENOID OPERATED PILOT VALVE) in place, causing the discharge of 100 gallons of AFFF onto the deck. 5 gallons of this then went into San Diego bay. NBSD Environmental and FRT responded to the spill however, it dissipated quickly and was unrecoverable A ship's message was sent out. Ship's personnel will review procedures to avoid accidental discharge of the fire suppression system. Violation U eSMR
1107661 02/23/2022 Surface Water Pier 13: A vessel reported sighting of oil sheen, with a fuel odor, source of sheen unknown. The sheen was confined to oil boom around ship and clean up was initiated. 300 gallons oily waste was recovered No sheen escaped the boomed area. Initial clean-up response was conducted by ship personnel and the NBSD Facility Response Team (FRT). A second oil containment boom was deployed around the ship and pier, and the oil was recovered using absorbent materials and skimmers. A ship's message was sent out. 1107672 09/29/2021 Surface Water Pier 3: A vessel was performing a routine planned maintenance check when a system component failed and caused the release of 924 gallons of lube oil. The Navy Port Operations Spill Response team was dispatched for clean-up and recovery. A second oil containment boom was deployed around the ship and pier, and the lube oil was recovered using absorbent materials and skimmers. A ship's message was sent out. 1093683 06/18/2021 Surface Water Pier 6: While operating the stern door, a hydraulic hose failed and spilled approximately 1.5 gallons of hydraulic oil in the water. The leak was stopped and contained. The spill occurred within the ship's oil containment boom. Absorbent material were applied and the spilled hydraulic oil was recovered. Violation U eSMR
1093682 06/11/2021 Surface Water Mole Pier: After re-fueling a SWRMC Diver's 60 foot vessel, sheen was observed on the water adjacent to the vessel. An investigation identified a pin-hole in the top portion of the fuel tank and the side of the hull due to corrosion, so when the tank was filled, diesel fuel was released from the tank. The hole in the tank was patched with a temporary patching material to stop the spill, then spilled fuel was recovered with absorbent pads. The fuel tank was de-fueled on the morning of 6/14/2021, so that the tank can be repaired permanently or replaced. Violation U eSMR

1093685 05/18/2021 Surface Water Pier 3: At approximately 0900 PDT, Naval Base San Diego (NBSD) Liquid Cargo member spotted an oil sheen from unknown source in the vicinity of Pier 3. NBSD FRT arrived on scene to investigate the amount and source of the spill. The material was discovered underneath and around the pier in the water. Unknown cause, unknown source. This spill was previously reported to the SDRWQCB on 05/19/2021 and subject of NOV R9-2021-0149 Area was boomed and pipes under the pier were also checked for proper pressurization. No discrepancies were identified with the pier piping systems. NBSD FRT spill response team was notified and responded for cleanup. Absorbent pads were deployed to recover the spill - approximately 300 gallons were recovered. Violation U eSMR
1090340 04/16/2021 UAUTHDISC On April 16, 2021, the Navy reported an unauthorized discharge of 1,300 gallons of diesel fuel to San Diego Bay due to a malfunctioning tank level indicator. Violation B Report

There did appear to be an issue with the Fuel Oil Control Console and having inaccurate TLI level readings for group 5. The ship reset the fault twice during the refueling and proceeded on, contrary to authorized procedures, prior to the spill occurring. CDR 23 citied training, incomplete watch bill, and lack of procedural compliance (human error) as the primary factors for the spill. This spill was previously reported to the SDRWQCB on 04/08/2021 and subject of NOV R9-2021-0149. The FRT spill response team was notified, responded to the spill, and recovered approximately 2180 gallons of fuel. Repair the faulty TLI. Conduct additional training to ship's personnel. 1093673 03/15/2021 Surface Water Pier 4: Ship was returning to port when a hydraulic fluid leak was identified from the ship's propulsion system (equipment failure). The motor for the leaking prop shaft seal was turned off to limit oil discharge. The FRT spill response team was notified prior to ship arrival to NBSD and immediately deployed an oil containment boom around the ship and deployed absorbents in an attempt to recover spilled oil. 1093684 03/03/2021 CTOX Chronic Toxicity-Sea Urchin or Sand Dollar-Larval develop Daily Maximum limit is 0 Pass/Fail (Pass = 0, Fail = 1) and reported value was 1 Pass/Fail (Pass = 0, Fail = 1) at NBSD-152. An investigation to the cause of the exceedance did not identify a source. The Stormwater Discharge Visual Observation form completed by the MMEC sampling team did not note any visual evidence of pollutants at the time the sample was collected. Industrial High Risk Facility operators perform pre-rain inspections before storms and NBSD Environmental personnel inspect all piers at least weekly. No sources of contaminants or best management practices (BMPs) deficiencies were observed on Pier 4 before the storm event. Evaluation of the BMPs associated with Outfall 218-247 (NBSD-152/Pier 4) is now being conducted concurrently with and in addition to weekly pier inspections. Sources of contaminants of concern will be evaluated in an effort to reduce toxicity in storm water runoff and to facilitate modifications to the Storm Water Pollution Prevention Plan as necessary. NBSD will continue to monitor the situation for High Risk Industrial Storm Water Areas (NBSD Permit, Attachment E, Section V.B.). Violation U eSMR

1093675 09/10/2020 Surface Water Pier 10: Spill of 25 gallons of lube oil, mixed with detergent used to clean engines into the Bay during an oily waste transfer. The cause appeared to be operator error/misalignment of hose connections. The spill was contained within the ship's oil containment boom. Ship personnel called CDO. FRT (Port Ops) responded - the spilled waste oil was fully recovered. A SITREP was released. 1093691 08/30/2020 CAT2 Zinc, Total Recoverable Maximum Daily (MDEL) limit is 95.1 ug/L and reported value was 110 ug/L at NGD-003. The caisson was evaluated for sources of zinc however no sources could be identified. All zinc anodes were replaced with aluminum within the caisson at the time of the last overhaul. NBSD will continue to assess the caisson ballast tank for potential sources, and closely monitor future results for zinc. Violation U eSMR
1093686 07/23/2020 Surface Water Mole Pier: The MPO crane maintenance team was filling the auxiliary diesel fuel tank on the floating crane TD260 and the tank over flowed. Diesel fuel spilled out onto the deck of the barge and into the water. The discharge was contained and initial notifications made to the spill response team. The spill response team recovered the fuel spilled to the water and MPO personnel cleaned up the fuel spilled on the deck of the vessel. A SITREP was prepared and distributed. The crane maintenance team will review fueling safety procedures and train personnel on lessons learned from this incident. Violation U eSMR
1093678 07/17/2020 Surface Water Chollas Creek Channel: A car that was parked in a parking space along the edge of the southern side of Chollas Creek Channel rolled past a parking block into the water which caused a release of engine coolant and motor oil into the water. The vehicle was removed from the water using a tow truck. The vehicle was removed from the water. The NBSD FRT Spill Response team was notified and responded; however, the spill was not recoverable. 1093676 07/15/2020 Surface Water Pier 2: Sheen caused by emergency dewatering process from a ship fire between July 15 and 17, 2020. The vessel was triple boomed and the floatable portion of the dewatering discharge was contained within the booms. Sheen was sighted within the booms on multiple consecutive days. AFFF firefighting water/foam was also released to the bay during the emergency incident. Initial reports were that the oily sheen was not recoverable. A massive team of first responders were on scene throughout the incident including the NBSD FRT (Port Ops) spill response team and a contracted spill response team. The spill response team was able to recover approximately 36 gallons of oil that spilled to the bay within the ship's oil containment boom during emergency firefighting activity and emergency dewatering using rubberizer. No preventative measures were identified because the spill occurred during and because of an emergency situation (ship fire). Violation U eSMR
1093689 07/10/2020 CAT2 Nickel, Total Recoverable Monthly Average limit is 6.8 ug/L and reported value was 20 ug/L at NGD-004. No source of nickel could be identified. In October 2020 a modification project began at the NGD. Included in the NGD modification scope of work is a replacement of all pumps and pressure regulators on the fire suppression system. 1078753 04/30/2020 CAT2 Nickel, Total Recoverable Monthly Average limit is 6.8 ug/L and reported value was 18 ug/L at NGD-004. Following receipt of the monitoring results, the NGD emergency fire suppression and salt water supply system was evaluated to determine sources of elevated nickel in the discharge. Based on the evaluation, no uncontrolled sources of nickel were identified. A review of intake sample results from samples collected from San Diego Bay during the March 9, 2020, docking event suggest that a significant source of the nickel in the NGD 004 discharge is from the source water (San Diego Bay). Although the NGD emergency fire suppression and salt water supply system has been repaired several times to prevent discharge to San Diego Bay under normal operating conditions, the component of the system that prevents discharge has failed shortly after each repair. NBSD continues to evaluate options to repair the system to operate without discharge to San Diego Bay. Violation A eSMR
