1118689 05/31/2023 CAT2 Chromium (VI) Dissolved Daily Maximum limit is 16 ug/L and reported value was 28 ug/L at EFF-001. Exceedance qualifies as upset due to laboratory inconsistency, and confirmation sampling under effluent limit. Currently investigating alternative laboratories. Violation B eSMR
1118164 05/31/2023 CAT2 Chromium (VI) Dissolved Monthly Average limit is 16.0 ug/L and reported value was 28.0 ug/L at EFF-001. We are currently investigating potential laboratory error. Violation B eSMR
1115855 03/31/2023 CAT2 Chromium (VI) Dissolved Monthly Average (Mean) limit is 4.9 ug/L and reported value was 39 ug/L at EFF-001. Currently investigating causes and future preventative measures. Violation B eSMR
1115853 03/13/2023 CAT2 Chromium (VI) Dissolved Daily Maximum limit is 4.9 ug/L and reported value was 39.0 ug/L at EFF-001. Immediately shut down system upon notification of exceedance. Currently investigating the cause of the exceedance. Violation B eSMR
1115854 01/31/2023 CAT2 Copper, Total Recoverable Monthly Average (Mean) limit is 6.6 ug/L and reported value was 18 ug/L at EFF-001. Currently investigating causes and preventative measures. Violation B eSMR
1113591 01/17/2023 CAT2 Copper, Total Recoverable Monthly Average (Mean) limit is 6.6 ug/L and reported value was 18 ug/L at EFF-001. We are backwashing the sand filters to increase solids capture Violation B eSMR
1112537 12/31/2022 CAT2 Copper, Total Recoverable Monthly Average (Mean) limit is 6.6 ug/L and reported value was 7.6 ug/L at EFF-001. We are backwashing the sand filters to increase solids capture Violation B eSMR
1110641 10/30/2022 CAT2 Copper, Total Recoverable Monthly Average limit is 6.6 ug/L and reported value was 9.68 ug/L at EFF-001. Adjust flocculant dosage, thoroughly clean clarifier and sand filters, etc. to reduce suspended copper. Violation B eSMR
1109738 09/27/2022 ATOX Acute Toxicity-Fathead Minnow-survival 4-Day Average (Mean) limit is 90 % effluent and reported value was 60 % effluent at EFF-001. *** MMP Exempt Reason:The Discharger has effluent limitation for toxic chemicals. GHD is in the process of evaluating the groundwater extraction and treatment system and is obtaining additional samples for toxicity testing. Operations and maintenance records indicate that flocculant dosage was increased by 6% (to 12%) to manage chromium concentrations on September 13, 2022. Following the toxicity failure, the flocculant was decreased back to 6%. Other chemicals, such as the acid and hydroxide to manage pH were not modified and the pH of the effluent was within permit limits, therefore those chemicals were ruled out. Copper, chromium, and total dissolved solids are not higher in samples that failed toxicity tests, suggesting that they are not the cause of toxicity. The cause continues to be under investigation. Violation B eSMR
1105709 05/31/2022 CAT2 Copper, Total Recoverable Monthly Average limit is 6.6 ug/L and reported value was 9.67 ug/L at EFF-001. Steps to investigate the cause of this exceedance and to prevent it in the future are currently in progress. Violation B eSMR
1104447 04/30/2022 CAT2 Copper, Total Recoverable Monthly Average limit is 6.6 ug/L and reported value was 9.97 ug/L at EFF-001. Steps to investigate the cause of this exceedance and to prevent it in the future are currently in progress. Violation B eSMR
1103126 03/31/2022 CAT2 Copper, Total Recoverable Monthly Average limit is 6.6 ug/L and reported value was 6.93 ug/L at EFF-001. Steps to investigate the cause of this exceedance and to prevent it in the future are currently in progress. Violation B eSMR
