Acute Toxicity Sampling was collected by Ironhouse Staff and relinquished to the FGL Lab Courier on February 7, 2023 at 1350 hours. Subsequently the sample was relinquished several more times prior to being received by ABC Labs on February 7, 2023 at 1700 hours. Apparently a janitor accepted delivery of the sample after normal business hours, thus preventing the analysis from being completed within a normal hold time.
Ironhouse staff were notified of the error on March 14, 2023. Therefore, resampling could not be completed within the required monitoring period and no results were reported. |
The contract laboratories are aware of the error. A letter from the lab is attached. |

The Influent BOD Sample was collected and custody taken by a contract Laboratory, but the contract Laboratory mislabeled the bottle and did not catch the mistake in time to analyze the sample (out of hold time) nor allow the District staff to re-sample within the week. |
The Laboratory is implementing a new labeling system as described in the attached letter from FGL Laboratories. |
