1148432 08/04/2025 Surface Water HCTP received a FAIL result on the bioassay sample taken on 8/4/25, 8/6/25, and 8/8/25 for the sample taken downstream of the HCTP discharge (RSW-002D). Isabella Suarez notified of FAIL result and HCTP initiated accelerated testing at the site per NPDES requirements. Please see non-compliance page in "attachment" section of this report for specifics related to the event. Accelerated testing sampling event generated three successive PASS results, putting HCTP back into compliance with its NPDES permit. Violation U eSMR
1140149 02/29/2024 CAT1 Sulfate, Total (as SO4) Monthly Average (Mean) limit is 250.0 mg/L and reported value was 275.0 mg/L at EFF-005. Violation B Report
1111240 09/07/2022 Surface Water The unprecedented heat wave experienced in our area during the month of September, created abnormally high temperatures in our influent, effluent, and both upstream and downstream sample sites. As a result, a temperature of 83 degrees F was documented at the HCTP downstream site, RSW-002D (formerly R-2). This is above the 80 degree F limit listed in NPDES Order no. R4-2019-0137. Please see non-compliance page of the September, 2022 report in the "attachment" section of the report for a detailed discussion of the situation. Wait for heat wave temperatures to subside to allow for more typical temperatures to be documented in the area. Violation B eSMR
1108442 07/12/2022 CAT2 Chlorine, Total Residual Daily Maximum limit is 0.1 mg/L and reported value was 2.6 mg/L at EFF-005. Third party SCADA Integrators will work more closely with facility staff when performing the upgrades to prevent a similar situation from occurring during the rest of the transition period. 1106534 05/31/2022 CAT2 Copper, Total Monthly Average (Mean) limit is 6.0 ug/L and reported value was 6.9 ug/L at EFF-005. Facility staff notified Pretreatment personnel of the violation. Lab staff also discussed matter with Operations staff to determine whether or not the violation was a process issue. The dosage of the chemical Metalsorb was increased and weekly accelerated monitoring of the site began. Three successive copper results under the limit were achieved and monitoring stopped. Dates and results of the accelerated monitoring as follows: 6-1-22 = 3.6 ug/L 6-9-22 = 1.8 ug/L 6-15-22 = 4.3 ug/L Violation B eSMR
1096234 08/31/2021 CAT2 Cyanide, Total (as CN) Monthly Average limit is 4.2 ug/L and reported value was 6.4 ug/L at EFF-005. *** MMP Exempt Reason:pending tolling agreement. Cyanide profiles were started at the plant in an effort to determine where the Cyanide was coming from. Sample profiles were collected weekly. In addition HCTP staff met to discuss any changes in the process that could contribute to the exceedance. There have been no changes to the process. Pretreatment staff were notified as there was one instant where a "hit" was received at the Influent. Pretreatment staff investigated industrial area. Nothing to report. Three successive results below our limit were achieved in October. Violation B eSMR
1089767 02/28/2021 CAT2 Copper, Total Monthly Average limit is 6 ug/L and reported value was 9 ug/L at EFF-005. *** MMP Exempt Reason:pending tolling agreement. HCTP Operations staff and Pretreatment personnel were notified of the violation and asked to review practices and be vigilant during inspections in the industrial community. Operations staff have increase the dosage of the Metalsorb chemical in the process in an effort to remove more metals. Violation B eSMR
