1145321 06/13/2025 OEV Enterococci Instantaneous Maximum limit is 110 CFU/100 mL and reported value was 200.5 CFU/100 mL at EFF-001. no action was taken as the next two results were <1.0, and 1.0 CFU/100mL. Violation B eSMR
1145322 06/07/2025 DMON The Enterococci sample for the week of 6/7/2025 was not taken by the District's onsite laboratory due to scheduling oversight. Laboratory was reminded that sampling schedules are all listed in the NPDES and are summarized in a shared file for ease of checking created by the District. Violation B eSMR
1142376 02/08/2025 CAT1 Settleable Solids Weekly Average limit is 1.5 ml/L/hr and reported value was 1.6 ml/L/hr at EFF-001. Try to get plant MLSS lower in preparation of high flow conditions. Also, lowered the high flow alarm. Violation B eSMR
1142374 02/07/2025 CAT1 Settleable Solids Instantaneous Maximum limit is 3.0 ml/L/hr and reported value was 6.5 ml/L/hr at EFF-001. Try to get plant MLSS lower in preparation of high flow conditions. Also, lowered the high flow alarm. Violation B eSMR
1142375 02/04/2025 CAT1 Settleable Solids Instantaneous Maximum limit is 3.0 ml/L/hr and reported value was 4.0 ml/L/hr at EFF-001. Try to get plant MLSS lower in preparation of high flow conditions. Violation B eSMR
1142373 01/31/2025 OEV Total Coliform Monthly Median limit is 70 MPN/100 mL and reported value was 110 MPN/100 mL at EFF-001. Manual FOG and foam removal as frequently as possible until conditions improved. Clarifier flush cleaning. Violation B eSMR
1142371 01/14/2025 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 1600 MPN/100 mL at EFF-001. FOG and foam were manually removed from the basins, and a clarifier flushing was conducted. Manual removal of grease was conducted daily and multiple times a day until conditions improved. Violation B eSMR
1142372 01/07/2025 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 280 MPN/100 mL at EFF-001. Manual removal of foam and grease from all basins with a vac trailer were conducted as often as possible until conditions improved. Clarifier flush cleaning was also conducted. Violation B eSMR
1139355 11/12/2024 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 1600 MPN/100 mL at EFF-001. No other testing results or observation indicated an issue that would have contributed to this exceedance. Results before and after exceedance were normal. Violation B eSMR
1139354 10/29/2024 DMON Contracted laboratory states that coliform test sample on this day was; invalidated due to an accident during the transferring steps of the analysis. Laboratory error. Sample was invalidated while being conducted. Violation B eSMR
1139353 10/08/2024 DMON Contracted laboratory stated; unable to quantify results due to insufficient/excessive oxygen depletion and QC failure during the initial analysis and the reset analysis. This effected both the INF & EFF BOD samples. Laboratory error. There has not been an issue since or prior to this one in terms of BOD. Violation B eSMR
1134943 09/03/2024 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 1600 MPN/100 mL at EFF-001. A temporary chlorine injection system was established just downstream of the normal injection site, and the leak was repaired without any loss of chlorine residual. Violation B eSMR
1130202 04/25/2024 CAT2 Chlorine, Total Residual Daily Maximum limit is 408 ug/L and reported value was 490 ug/L at EFF-001. Accelerated monitoring was conducted with no other issues. The chlorine analyzers were immediately corrected and the target residual of bisulfite was increased. Violation B eSMR
1126585 03/13/2024 CAT2 Chlorine, Total Residual Maximum Daily (MDEL) limit is 408 ug/L and reported value was 620 ug/L at EFF-001. Bisulfite residual was lower than target but this shouldn¿t have caused an issue. I suspect that this issue is a combination of partial mixing between injection and sample locations, and issues with the programming of the system needing to be tuned differently for different flow rates, but it is difficult to dial this in when the flows are changing so much from rains. We are working on this tuning and increasing maintenance to the system. target residual was increased. Violation B eSMR
1126583 02/29/2024 OEV Flow Monthly Average (Mean) limit is 2.200 MGD and reported value was 2.436 MGD at INF-001. *** MMP Exempt Reason:This violation is not an effluent limit. By happenstance, the district already had flowmeters installed in suspected infiltration locations around town in order to best plan the next locations for slip-lining prior to these storms. Violation B eSMR
1129290 02/10/2024 CAT1 Settleable Solids Weekly Average (Mean) limit is 1.5 ml/L/hr and reported value was 1.75 ml/L/hr at EFF-001. Violation B Report
1126584 02/06/2024 CAT1 Settleable Solids Instantaneous Maximum limit is 3.0 ml/L/hr and reported value was 10.0 ml/L/hr at EFF-001. The clarifiers were monitored all night while operators manually controlled the plant in the highest stage of emergency operations we have developed. Solids leaving the clarifiers only occurred for a brief moment while emergency operations were being conduct, which was incredible considering the level of hydraulic loading. Violation B eSMR
1126582 01/09/2024 CAT2 Chlorine, Total Residual Maximum Daily (MDEL) limit is 408 ug/L and reported value was 1200 ug/L at EFF-001. Accelerated monitoring was conducted and bisulfite analyzers were checked and serviced. No other issues occurred immediately and nothing was found wrong with the analyzer. Changes were made to the programming settings. Violation B eSMR
1123478 11/14/2023 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 920 MPN/100 mL at EFF-001. This sample result was not reported to us until 16 days after the sample was collected. It was reported to us with the following week's coliform sample which was 2.0. because of this, no significant changes were made. Violation B eSMR
1123477 10/11/2023 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 350 MPN/100 mL at EFF-001. Elevated NTUs were from running the plant in a way as to control filamentous bacteria growth, which was successful. This operation style was no longer needed and discontinued in mid November. There was an issue with the chlorine surging instead of flowing that was intermittent and took until late December to identify and correct. Not sure if that had anything to do with the coliform results as this problem was very mild and only at very low flows. Violation B eSMR
1121437 08/15/2023 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 540 MPN/100 mL at EFF-001. Chlorine dosage was increased by 1.2 mg/L Violation B eSMR
1121435 07/31/2023 OEV Total Coliform Monthly Median limit is 70 MPN/100 mL and reported value was 109.5 MPN/100 mL at EFF-001. Difficult to make corrections when sample results take 8-15 days, but we switched to a laboratory that gives results in 4-6 days. collecting additional samples will be better organized in the future. Violation B eSMR
1121436 07/18/2023 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 240 MPN/100 mL at EFF-001. Wasting was increased to lower plant MLSS and water quality improved over the next 10 days until effluent constituents were back to normal levels. Violation B eSMR
1121438 07/05/2023 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 540 MPN/100 mL at EFF-001. No action was taken other than reviewing data trends and checking equipment and infrastructure maintenance. Violation B eSMR
1118805 05/08/2023 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 540 MPN/100 mL at EFF-001. Increased cleaning of the chlorine probes and bleeding feed chlorine lines from the tanks to release built up gases. Not sure if it had any effect but should make for smoother operation regardless. Violation B eSMR
1115975 03/15/2023 CAT1 Settleable Solids Instantaneous Maximum limit is 3.0 ml/L/hr and reported value was 14.0 ml/L/hr at EFF-001. Was able to mitigate by running the plant manually. Accelerated monitoring was conducted for a week after the event with out any other exceedances. Violation B eSMR
1109611 09/30/2022 OEV Total Coliform Monthly Median limit is 70.0 MPN/100 mL and reported value was 76.5 MPN/100 mL at EFF-001. increased target chlorine residual to not have elevated sample results in case of sample spikes. Violation B eSMR
1109610 09/19/2022 OEV Total Coliform Instantaneous Maximum limit is 230.0 MPN/100 mL and reported value was 1600.0 MPN/100 mL at EFF-001. Increased frequency of grounds keeping and cleaning in the close vicinity of the chlorine contact basin. Violation B eSMR
1092274 05/24/2021 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 1600 MPN/100 mL at EFF-001. Target Chlorine residual was increased. Violation B eSMR
1089283 02/17/2021 OEV Total Coliform Instantaneous Maximum limit is 230 MPN/100 mL and reported value was 1600 MPN/100 mL at EFF-001. Review Laboratory procedures, and make sure operations & laboratory coordinate. Violation B eSMR
