1109608 09/06/2022 DMON During the month of September 2022, we had our historian go down for a period of 6 days. We have been attempting to recover data from the PLC for dates 9/6-9/11/2022 but are unable to do so. The lost reportable data includes: UV Transmittance Average UV Transmittance Maximum UV Dose Average UV Dose Maximum We have alarm histories that are accurate as well as daily documented values for each one of these data points that have been reported. It would be a one-time read instead of a 24-hour max or average value. We have added an alarm into our SCADA call out system that will notify us if or when the historian goes down. In addition, We will be installing a stand-alone system as a backup to help prevent any future incidents of lost data. Violation U eSMR

Jacobs repaired the cap and the plumbing from the air header to the diffusers and unclogged a majority of the diffusers by Monday August 26th on both basins. We also planned a full diffuser replacement and overhaul of the diffusers for February/March 2023. The new diffusers are on order and will not be available until late January or early February. 1106722 06/29/2022 CAT1 Ammonia, Total (as N) 1-Hour Average (Mean) limit is 37 lb/day and reported value was 55.1 lb/day at EFF-A. To prevent further exceedances, Jacobs is running the centrifuge longer hours but at a lower flow rate. We have turned on our third aeration blower and are running it at 100%. In addition to the longer run hours and third blower running the City of Clovis has reduced flows to the facility down to 1.6 MGD as a daily average. The Centrifuge is scheduled to be repaired early August. While the centrifuge is being repaired, we will have a belt press rental on site to prevent excessive inventory in our aeration basins. Jacobs and the City of Clovis will collaborate together and determine the best time to repair the diffusers. Violation U eSMR

1100007 12/20/2021 CAT2 Cyanide, Total (as CN) Maximum Daily (MDEL) limit is 35 ug/L and reported value was 42 ug/L at EFF-A. Monitoring will continue Violation A eSMR
1100008 12/20/2021 CAT2 Cyanide, Total (as CN) Monthly Average limit is 17 ug/L and reported value was 42 ug/L at EFF-A. Will continue to monitor. Violation A eSMR
1080702 07/16/2020 DMON Permit requires quarterly monitoring of total hardness to be collected concurrently with the metals samples. Hardness at location RSW-001U (upstream Fancher) was inadvertently missed when metals were sampled for in July. There was no measurable flow in August and September to resample. New staff trained and QA/QC measures implemented. Violation U eSMR
1075218 04/28/2020 OEV Total Coliform Instantaneous Maximum limit is 240 MPN/100 mL and reported value was 1011.2 MPN/100 mL at UVS-001. Sample collection procedures have been reviewed with staff as a preventative measure. Violation U eSMR
1072140 02/20/2020 OEV Turbidity Instantaneous Maximum limit is .5 NTU and reported value was .92 NTU at FIL-001. *** MMP Exempt Reason:Not an effluent violation. It is an other violation type and considered Order Condition violation. The high turbidity event may have been caused by the two membrane tanks being in standby for an extended period of time and not being recirculated prior to pumping to the filtrate header. A rung of programming code was found that could have potentially allowed filtration directly following an extended standby period. An inhibitor was previously added to the filtration logic code, but found that logic triggering "Normal Filtration" did not include the inhibitor. 5 minutes of standby has been added to the inhibitor code of the "Normal Filtration" logic to assure the filtrate pump will not run directly after an extended standby period unless 10 minute recirculation has completed. Violation B eSMR
1070175 12/12/2019 CTOX Chronic Toxicity Other limit is 1.3 TUc and reported value was 16 TUc at EFF-A. Given the high variability in both tests, we are requesting to repeat the study to confirm these findings. We will clean our effluent pipe after the UV. The study would then be repeated. It is anticipated the cleaning and repeat testing can be performed over the next 14 to 21 days. Violation U eSMR
1069801 12/11/2019 CAT2 Chlorine, Total Residual Instantaneous Maximum limit is .01 mg/L and reported value was .05 mg/L at EFF-A. *** MMP Exempt Reason:According to the discharger no chlorinated effluent was discharged from the facility. This is a violation of order condition not an effluent violation. A sample was taken from the recycled water tank at 9:46 AM and the result was <0.0015 mg/L . Please note that no chlorinated effluent was discharged from the facility. Violation B eSMR
