1109608 09/06/2022 DMON During the month of September 2022, we had our historian go down for a period of 6 days. We have been attempting to recover data from the PLC for dates 9/6-9/11/2022 but are unable to do so. The lost reportable data includes: UV Transmittance Average UV Transmittance Maximum UV Dose Average UV Dose Maximum We have alarm histories that are accurate as well as daily documented values for each one of these data points that have been reported. It would be a one-time read instead of a 24-hour max or average value. We have added an alarm into our SCADA call out system that will notify us if or when the historian goes down. In addition, We will be installing a stand-alone system as a backup to help prevent any future incidents of lost data. Violation U eSMR

Hardness at location RSW-001U (upstream Fancher) was inadvertently missed when metals were sampled for in July. There was no measurable flow in August and September to resample. 1072140 02/20/2020 OEV Turbidity Instantaneous Maximum limit is .5 NTU and reported value was .92 NTU at FIL-001. *** MMP Exempt Reason:Not an effluent violation. It is an other violation type and considered Order Condition violation. The high turbidity event may have been caused by the two membrane tanks being in standby for an extended period of time and not being recirculated prior to pumping to the filtrate header. A rung of programming code was found that could have potentially allowed filtration directly following an extended standby period. An inhibitor was previously added to the filtration logic code, but found that logic triggering "Normal Filtration" did not include the inhibitor. 5 minutes of standby has been added to the inhibitor code of the "Normal Filtration" logic to assure the filtrate pump will not run directly after an extended standby period unless 10 minute recirculation has completed. To prevent this type of turbidity spike in the future, two tanks will be placed in auto sequence off and two tanks in auto sequence available. Having two tanks auto sequence off will keep the same two membrane tanks running instead of the tanks rotating between filtration and stand-by. We will also investigate changing the SCADA program to have the tanks go into recirculation after coming out of standby to ensure turbidity is below 0.2 NTU before filtration mode is initiated.
