1136568 10/18/2024 DMON The laboratory staff member responsible for performing weekly EFF-001 sampling began an indefinite leave on October 11, 2024. Weekly EC samples were missed while acclimating to their departure. The facility has chosen to restructure the sampling and analysis procedures such that WWTP operators are assisting laboratory personnel with non-regulatory sampling. The Facility has also reduced the amount of sampling needed for process control data collection. These temporary accommodations made by operations personnel will allow the remaining laboratory staff to focus on regulatory sampling while adjusting to the decrease in laboratory staffing. The corrective action was implemented on October 22, 2024. Violation U eSMR

1131431 07/17/2024 Surface Water Turbidity samples collected at RSW-001 and RSW-002 on July 17 resulted in a 1.1 NTU difference exceeding the 1 NTU receiving water limitation (Section V.A.18.b.). Operators will monitor turbidity readings from the chlorine contact basin and reduce flows through the filters if elevated readings are observed. Excess discharged will be conveyed to a storage pond. 1122105 10/11/2023 CAT2 Chlorine, Total Residual 1-Hour Average (Mean) limit is 0.019 mg/L and reported value was 0.52 mg/L at EFF-001. The programmed automatic chlorine meter bump tests have been removed. Instead an operator will manually bump test the chlorine meters. This means an operator will be present to visually assure that no chlorinated effluent will be discharged during a bump test. Violation B eSMR
1121283 09/20/2023 DMON Dissolved Oxygen and Turbidity were not sampled a second time in this monitoring period. Laboratory staff forgot to complete monitoring. In the future, prior to accessing the programming (Programmable Logic Controllers or SCADA) involving the chemical dosing systems, the pond 6 gate valve will be opened, diverting effluent flow from the river to Emergency Retention Basin 6. This will eliminate the possibility of any control glitches causing an Effluent violation. Violation B eSMR

1109450 09/30/2022 CAT2 Dichlorobromomethane Monthly Average limit is 16 mg/L and reported value was 18.7 mg/L at EFF-001. Operations staff decreased Chlorine dosing last month. The first sample taken on September 9, 2022 remained above the AMEL. In response to this operations staff adjusted the operation schedule for the cloth media filters, increasing the number of filter backwash cycles, which should lower the amount of organic solids that are present at the time of chlorine introduction. Which should decrease the formation of Dichlorobromomethane. Violation U eSMR
1108556 08/31/2022 CAT2 Dichlorobromomethane Monthly Average (Mean) limit is 16 mg/L and reported value was 18.3 mg/L at EFF-001. Operations staff has lowered the Chlorine does to decrease the occourance of Dichlorobromomethane. Violation B eSMR
