BOD sample that was taken on 07/23/2026 all samples in dilution series depleted remaining oxygen more than 1 mg/l causing the entire sample to be invalid. Manager used highest value to get a result of >=21.4 mg/l. Which was used for loading and percent removal calculations. I would also like to note that BOD on Secondary Clarifiers (Prior to the chlorine contact channels and prior to the EFF-001 location) for process control came back as a 16.5 mg/l for 7/26/26. So, I do not feel that the sample would be greater than the 21.4 mg/l. |
Lab manager is looking into this further but at this current time there is no real indication of what happened with the sample. |

Operations has cleaned the collection vault which feeds the Effluent box. In house lab is currently running side by side analysis with contract lab. As well as in house lab is working toward accreditation. Contract lab has been instructed to increase turnaround time for a faster response time. |

The influent composite sampler malfunctioned. Due to miscommunication another composite was not run causing us to miss the 13 day sampling period. |
A back up composite sampler is being outfitted and prepared to swap in case an incident like this would ever happen again. As well as a clear communication protocol and a sampling checklist that entire staff can see to help assist with any missed sampling. |

There was a high arsenic value in the first quarter of sampling which exceeded the monthly average as well as the daily maximum limit. Sludge hauler sampling that also takes place during that time shows a non-detect. |
Staff will continue to monitor. Believe to be an anomaly as the hauler also takes sample biannually and had a non-detect for the same month of sampling. All other samples for the year were below limits. |

Staff contact MBAS and review with CAWD¿s Laboratory staff the analytical results and requested that all permit required TSS samples be analyzed the same day when the samples are obtained.
The second corrective action occurred on October 18, when Parkson Corporation completed the refurbishment of the Dyansand filters as pretreatment to the MF/RO process and placed the units back on-line while starting the Cerium Chloride coagulant.
