The discharger has not identified a specific cause but states the GAC treatment system failed. A full diagnosis of the treatment system is still underway. Potential corrective measures include, GAC vessel layout changes, GAC backwashing routing operations and replacing GAC in one or both vessels. The discharger will conduct start-up phase monitoring when corrective actions are implemented. Violation B Report

The discharger has not identified a specific cause but states the GAC treatment system failed. A full diagnosis of the treatment system is still underway. Potential corrective measures include, GAC vessel layout changes, GAC backwashing routing operations and replacing GAC in one or both vessels. 1090660 05/21/2021 DMON The discharger did not perform standard observations nor collect receiving water samples for DO, hardness, and salinity when performing effluent confirmation sampling. (see Attachment E Table E-2 and section VIII for requirements). None were addressed by the discharger. Staff sent email on 5-28-21 asking for this data. The discharger was made aware of missing monitoring and it will implement necessary steps to comply in the future. Violation B Report

As preventive measures, the discharger reviewed chain-of-custody protocols, recirculated effluent to flush out potential stagnant zones in the media vessels, and replaced flexible hosing connecting GAC vessels. It did not perform standard observations nor receiving water sampling for DO, salinity, and hardness. Violation B Report
