1095934 07/24/2021 CAT1 Settleable Solids Instantaneous Maximum limit is 3.0 ml/L and reported value was 4.0 ml/L at EFF-001. Upon inspection, staff observed the mechanical sampler suction tube was laying on the floor of the effluent chamber when the high SS grab sample was collected. With the suction tube laying on the floor of the chamber, the mechanical sampler must have vacuumed up some solids resulting in the high recorded value. After this observation, the settleable solids grab sample has been taken directly from the effluent rather than through a mechanical sample device. In addition to sampling for settleable solids directly from effluent, staff reset the mechanical sampler suction tube so it is suspended in the effluent. Violation B eSMR
1101211 07/15/2021 DMON It appears the gap in continuous monitoring was due to a computer update. The update automatically restarted the primary operations computer at about 3:15 AM. After the computer restarted, it did not activate the SCADA system. At 6:55 AM an operator logged onto the computer and started SCADA, thus starting continuous monitoring logging again. We are making every effort to correct this problem and have engaged a SCADA integrator to improve data logging redundancy and assure there is logging consistency throughout the SCADA system. 1092649 06/13/2021 CAT2 Chlorine, Total Residual Instantaneous Maximum limit is 1800 ug/L and reported value was 4266 ug/L at EFF-001. The on-call operator called a second operator, and both drove to the plant. The first operator that arrived at the plant reset the pumps and brought C-water pumps online. After resetting the carrier water systems and the SBS residual was restored, the operators checked Plant operations and reviewed the SBS residual information. The operators responded quickly and appropriately. The quick response minimized the time the carrier water system was offline, and SBS residual was restored within minutes. Violation B eSMR

After seeing the operations room SCADA was not working as expected, staff brought utility power online and performed troubleshooting on the SCADA system. Also, as a precaution, an off-line secondary clarifier was placed into service so the secondary clarifier that had been online could be cleaned. The primary clarifier that was online was also swapped out for a clean primary clarifier. In addition, the chlorine dose rate was increased to intensify disinfection through the chlorine contact tank. As an added step, the chlorine contact basin was thoroughly cleaned. Going forward, the plant operators will continue to regularly discuss and evaluate lab results for a variety of treatment processes with the goal of staying in compliance with the fecal coliform limits. Upon investigation it was determined that the Y-strainer from the SBS chemical tanks had plugged.... The Y-strainer was cleaned, and [SBS] residual was regained at 4:53 am. Violation B eSMR
1073453 01/16/2020 CAT2 Chlorine, Total Residual Instantaneous Maximum limit is 1800 ug/L and reported value was 3550 ug/L at EFF-001. The steps we have taken to ensure this does not repeat itself is we have placed the cleaning of the [SBS] y-strainer in our maintenance management system so that a work order will be issued each month. We do rotate duty and standby pumps on a regular basis and it should be noted that the last time the standby pump was operated in worked well. Violation B eSMR
1066729 09/06/2019 CAT2 Copper, Total Recoverable Instantaneous Maximum limit is 840 ug/L and reported value was 1080 ug/L at EFF-001. As part of Table E-4 in the permit, accelerated monitoring was initiated. An additional composite sample was taken on November 6, 2019 and on November 15, 2019. The result of 840 ug/L was received by the contract lab on November 1st and read on November 2nd. It should be noted that on September 5th and September 7th, the results for Copper, Total Recoverable were a non-detect (ND) and a 6.02 ug/L respectively. The sample result taken on November 6th was a 9.7 ug/L, all showing the treatment plant is well within permit requirements. Violation U eSMR
1066731 09/05/2019 CAT1 Oil and Grease Instantaneous Maximum limit is 75 mg/L and reported value was 93.5 mg/L at EFF-001. As part of Table E-4 in the permit, accelerated monitoring was initiated. An additional grab sample was taken on November 6, 2019 and on November 15, 2019. The result of 93.5 mg/L was received by the contract lab on November 1st and read on November 2nd. It should be noted that the Oil and Grease result from the sample taken on September 6th was a non-detect (ND) and the result on September 7th was 21.0 mg/L. The sample taken on November 6th was also a ND showing the treatment facility is well within permit requirements. This result appears to be an outlier. We will continue to review our sampling protocols to ensure proper collection and handling of samples. Violation U eSMR
