1117517 04/19/2023 OEV Fecal Coliform Daily Maximum limit is 400 MPN/100 mL and reported value was 16000 MPN/100 mL at M-001. The pump was immediately placed in manual and District Operations staff communicated the failure to the electrical contractor. The District¿s electrical contractor troubleshot the failure, found the blown fuse, and then replaced the blown fuse. Once the fuse was replaced, District Operations staff placed the system back into normal operation and ORP levels began to return to normal. The permanent tie in of the communication cables (no splices) was completed by the District¿s electrical contractor later the same day at approximately 1:15 PM. Operations staff verified both chemical pumps were operational and receiving communication following the permanent tie in. The District does not foresee any future interruptions or repairs necessary to the communication cables now that the permanent cables have been tied in and all major excavations for the Redundancy Project are complete. Currently, District Operations staff perform a monthly inspection of the flash mixer but does not inspect the chemical feed piping internals where the blockage occurred. The internal inspection of the flash mixer injection piping has been added to the monthly work order to prevent reoccurrence of the blockage. Violation U eSMR

To prevent reoccurrence, the District will operate at this higher chlorine dosage for the foreseeable future and continue to monitor processes for any changes. In addition, the District has raised its polymer dosage for its chemically enhanced primary treatment. Violation B eSMR
1107734 07/14/2022 OEV Fecal Coliform Daily Maximum limit is 2000 MPN/100 mL and reported value was 2400 MPN/100 mL at M-001. I will be meeting with my Operations staff to update our maintenance procedure so that this does not occur again. In addition, maintenance and oil sampling frequency will be mitigated upon completion of the Redundancy Project. Once the District¿s current Redundancy Project is complete, the District will be able to fully bypass the FFR to perform maintenance and rehabilitation while maintaining full treatment of the waste stream. 1105881 05/19/2022 CAT2 Chlorine, Total Residual Instantaneous Maximum limit is 9.96 mg/L and reported value was 10.7 mg/L at M-001. The District is currently in the design phase of the Dechlorination System Improvement Project. This Improvement Project is for the relocation of the dechlorination injection point to make use of the entire volume of the chlorine contact tank effluent chamber to provide mixing and reaction time, increase mixing energy to ensure that Sodium Bisulfite is well mixed, and adequately apply Sodium Bisulfite to contact all effluent equally. Tentative completion of the Improvement Project is July 2022. Violation B eSMR
1098617 11/01/2021 CAT2 Chlorine, Total Residual Instantaneous Maximum limit is 09.96 mg/L and reported value was 11.3 mg/L at M-001. The cause of the exceedance is unknown, but may have been due to a high demand of chlorine at the chlorine contact tank. The high chlorine demand may have caused a heavier than normal load to the de-chlorination system and therefore causing the exceedance. All processes have returned to normal since the exceedance, but operations staff will continue to monitor the system to ensure proper operation. In addition, the District has developed a scope of services with a professional engineering firm to perform an assessment of the District's disinfection system. Services include the following; - Perform onsite inspection of the disinfection and dechlorination systems to document the existing equipment, its condition, connectivity, and control. - Investigate physical configuration of the effluent chamber and location of instrumentation. - Review available information on the existing control and instrumentation systems. - Prepare a Technical Memorandum describing the system and providing recommendations for improving control of the disinfection and dechlorination system. The report will discuss if control improvements alone are recommended, or if physical improvements may be necessary. - The Technical Memorandum will provide an assessment of additional potential improvements beyond revised loop control, such as improvements to physical configuration, location of probes, or additional features such as baffle or weirs, if warranted. Upon completion of the Technical Memorandum and at the District¿s request, the professional engineering firm will prepare a scope and fee to prepare Construction Plans, Specifications, and Estimates for the recommended improvements. Violation B eSMR
1096999 10/20/2021 OEV Fecal Coliform Daily Maximum limit is 2000 MPN/100 mL and reported value was 9200 MPN/100 mL at M-001. The District is in the process of performing a Local Limits Study to update its Pretreatment Program. In addition, the District has developed a scope of services with a professional engineering firm to perform an assessment of the dechlorination system and provide a subsequent technical memorandum. Operations staff have begun chemically enhanced primary treatment (CEPT) of both primary clarifiers. Operations staff also performed a hydraulic flush of the Fixed Film Reactor followed by the draining and cleaning of the Chlorine Contact Tank. In addition, weekly rinsing of the effluent weir has been added to Operations weekly work order tasks to limit growth that could potentially affect fecal coliform analysis. These processes should enhance the treatment efficiency and assist with preventing further exceedances. Violation B eSMR

The District will be updating its Standard Operating Procedure for performing CCT quarterly maintenance by changing the order in which the CCT serpentine channels are drained and rinsed. 1078132 07/30/2020 OEV Fecal Coliform Daily Maximum limit is 2000 MPN/100 mL and reported value was 2200 MPN/100 mL at M-001. To prevent reoccurence, the District is continuing chemically enhance primary treatment, higher dosage of sodium hypocrite and increased re-circulation rate at fixed film reactor. In addition, the District performed additional fecal analysis during the weekend following the violation to ensure maintained compliance. Violation B eSMR
1078131 07/09/2020 CAT2 Chlorine, Total Residual Instantaneous Maximum limit is 9.96 mg/L and reported value was 16.40 mg/L at M-001. To prevent reoccurrence, the District's Electrician/Integrator reprogrammed the Programmable Logic Controller (PLC) and tested the IP Address retention by cycling the power on and off. The PLC held its IP Address even after the power was cycled off. If there were to be future power interruptions, the PLC should not lose its IP Address or communication with the chemical pumps. 1077041 06/09/2020 OEV Fecal Coliform Daily Maximum limit is 2000 MPN/100 mL and reported value was 9200 MPN/100 mL at M-001. Operations staff increased the chlorine dosage at the chlorine contact tank to lessen bacterial activity. In addition, Operations staff have begun adding polymer to the primary clarfier to enhance settling in the clarifier and lessen TSS and BOD loading to our secondary and disinfection process. Operations staff continue to monitor all our processes and process values to ensure recurrence does not occur. Violation B eSMR
