1119735 07/17/2023 Order Conditions A dewatering pump hose discharged an estimated total of 7430 gallons of secondary treated effluent at a rate of 530 GPM for ~14 minutes. The majority of the spill was contained within the plant, however an estimated <500 gallons reached the Fallbrook Creek from an area other than the approved discharge point. After a thorough investigation, it is believed the hose was not correctly secured in place and a surge in the line led to the hose moving from intended location. Full report is attached and had been submitted on 07/18/23 Future needs for pumping will require thorough inspection from a second operator to ensure tubing is secured in place Violation U eSMR
1119017 06/29/2023 DMON The SMGTP went offline on 06/27/2023 at approximately 12:00 until 07/03/2023. During this time there was no Brine (M-003) discharge due to normal operations. However, operations failed to notify the lab of a mandatory flush of RO trains (16,000 gal) on 6/29/2023. This caused a failure to sample for required parameters on 6/29/2023. The SMGTP operators will be trained on proper notification procedures for plant startup/shutdown/flushing. Operators will be trained on sampling and field testing for short hold parameters. 1121303 04/01/2023 LREP Attachment E, section VI.A of the NPDES Order requires the District to prepare and submit a Climate Change Action Plan (CCAP) within three years of the effective date of this Order. The effective date of the NPDES Order was April 1, 2020. Therefore, the CCAP was due March 31, 2023. To date, the District has not submitted a CCAP. Violation B Report
1118020 12/31/2022 DMON Due to a clerical error, several of the parameters were missed during the semi-annual sampling period. These test include EPA Method 624, Chromium III, Chromium VI, and Tributyltin. These parameters are listed as performance goals per Order R9-2019-0169. 1066771 10/07/2019 OEV pH Daily Minimum limit is 6.0 SU and reported value was 5.85 SU at M-001. A re-sample of the effluent should have been analyzed on this day. In the future, the laboratory staff will re-sample anytime the laboratory pH probe is suspected to be reading low. Violation B eSMR
1062862 07/13/2019 CAT1 Settleable Solids Weekly Average limit is 1.5 ml/L and reported value was 1.7 ml/L at M-001. The plant staff member who performed the maintenance causing this exceedance will be re-trained. Violation B eSMR
1062863 07/12/2019 CAT1 Settleable Solids Instantaneous Maximum limit is 3.0 ml/L and reported value was 5.0 ml/L at M-001. The staff member will be re-trained on proper procedures in order to reduce future noncompliance. Violation B eSMR
